Overview: <p class="ignore-global-css">We are looking for a Direct Support Coordinator to join our team!</p> <p class="ignore-global-css">The Direct Support Coordinator acts as a key member of the I/DD Direct Support Team by providing direct support services and training to clients of Adjoin as well as assisting the case responsible person in overseeing the various aspects of client services. The purpose of this role is to promote the dignity, safety, independence, choices, rights and responsibilities of Adjoin clients through the execution of the DSC job responsibilities. The DSC also exemplifies Adjoin&rsquo;s vision and meets standards of quality, safety, satisfaction and goals of the client to optimize quality of life and independence.</p>
Responsibilities: <ul class="ignore-global-css"> <li class="ignore-global-css">Advance client&rsquo;s decision-making, productivity and participation in a range of daily activities. Facilitate the client&rsquo;s inclusion in positive community experiences.</li> <li class="ignore-global-css">Promote independence in daily activities yet be mindful of client&rsquo;s intellectual and physical disabilities and assist with meals, cooking, household maintenance, grooming and dressing, as assigned. Maintain current knowledge of client&rsquo;s needs.</li> <li class="ignore-global-css">Implement individual training and support programs under the direction of the case responsible person.</li> <li class="ignore-global-css">Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community.</li> <li class="ignore-global-css">Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.</li> <li class="ignore-global-css">Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required.</li> <li class="ignore-global-css">Provide positive advocacy when the individual&rsquo;s preferences, needs, or talents are neglected or overlooked.</li> <li class="ignore-global-css">Address challenging behaviors proactively and respectfully, using methods outlined in plans and training.</li> <li class="ignore-global-css">Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs.</li> <li class="ignore-global-css">Assist with training employees to perform their direct support duties in accordance with the policies and procedures of Adjoin.</li> <li class="ignore-global-css">Prepare and maintain current staffing schedules and coordinate new staff training schedules.</li> <li class="ignore-global-css">Be part of the on-call rotation as needed and required.</li> <li class="ignore-global-css">Monitor and report to case responsible person on client and employee activities.</li> <li class="ignore-global-css">Complete CSPs, monthly/quarterly reports, evaluations, intakes and assessments under the direction of the case responsible person.</li> <li class="ignore-global-css">Review activity logs of clients and report significant events or concerns to case responsible person.</li> <li class="ignore-global-css">Provide direct support at least 60% of logged time to ensure clients service is rendering per their CSP.</li> <li class="ignore-global-css">Perform all other job duties as assigned.</li> </ul>
Requirements: <ul class="ignore-global-css"> <li class="ignore-global-css">18 years of age or older.</li> <li class="ignore-global-css">Direct Support Coordinator</li> <li class="ignore-global-css">High School Diploma or Equivalent.</li> <li class="ignore-global-css">Bachelor&rsquo;s degree or 3 years equivalent experience and/or educational equivalent in working with individuals with disabilities.</li> <li class="ignore-global-css">Aptitude for confidentiality, organizational skills and good interpersonal skills.</li> <li class="ignore-global-css">Certification in an approved First Aid and CPR class; or successful completion within 45 days of the date of hire.</li> <li class="ignore-global-css">Insured, registered vehicle in safe working condition, and proof of current CA driver&rsquo;s license.</li> <li class="ignore-global-css">Adjoin&rsquo;s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.</li> </ul> Other Requirements:<br /> <ul class="ignore-global-css"> <li class="ignore-global-css">Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.</li> <li class="ignore-global-css">A Direct Support Coordinator works as scheduled by the Regional Manager and/or his/her designee. His/her schedule may be either part or full time, depending on the needs of the clients and Adjoin. Overtime may be required as dictated by operational needs; however, no holiday or overtime may be worked without the prior approval of the Direct Support Coordinator's supervisor.</li> <li class="ignore-global-css">Successful completion of pre-employment drug screen and background check.</li> <li class="ignore-global-css">Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.</li> <li class="ignore-global-css">Acceptable DMV record.</li> <li class="ignore-global-css">This position requires employees to be fully vaccinated and boosted against Covid-19.</li> </ul> Working Conditions:<br /><br />Some of our clients are physically impaired and require assistance in and out of wheelchairs and other assistive devices. The Direct Support Coordinator must be able to perform such tasks on an urgent or emergency basis. Many of the jobs/activities in which our clients engage require physical exertion on the part of the Adjoin staff person (i.e., walking (75% of workday), climbing stairs (10% workday but on a frequent basis), lifting (10% of workday but on a frequent basis), stretching, bending, etc.). A Direct Support Coordinator must have the ability to talk, hear, and see in order to assist the client in his/her quest for independence.<br /><br />The Direct Support Coordinator is responsible for the safety and well-being of a disabled client they are providing service for. Must be able to react appropriately and quickly in the event of an emergency such as client elopement, a natural disaster, fire or other unforeseen circumstance. An appropriate reaction might be having to follow or help the client evacuate in the event of a fire or emergency which may involve ascending or descending of stairs, walking a significant distance, or being able to summon emergency responders.<br /><br />Appropriate and professional dress is expected.<br /><br />The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Potential exposure to noisy environments at job sites. Exposure to blood borne pathogens could occur. (Hepatitis B vaccines are offered to all employees at no cost to the employee.)<br /><br />The Direct Support Coordinator must be capable of making on-site visits to clients. Often these on-site visits are at a distance from the office and no regular means of public transportation exists. Overnight travel is rare but may be required for training and as part of a participant's program. The Direct Support Coordinator is expected to perform duties assigned during hours needed to meet client needs, which means that the schedule may include evenings, weekends, holidays or other non-typical work hours.<br /> <p class="ignore-global-css">&nbsp;</p>
