Responsibilities: <p class="ignore-global-css"><strong class="ignore-global-css">Client Support:</strong></p> <p class="ignore-global-css">The Case Coordinator acts as a vital role within the Adjoin Veterans program. The purpose of this role is to provide exceptional services to the homeless Veteran population through advocacy and empowerment. The Case Coordinator&rsquo;s goal is to adhere the principles of the Housing First Model in conjunction with embracing Veteran&rsquo;s choice to obtain and maintain housing stability.</p> <p class="ignore-global-css">A Case Coordinator is focused on:</p> <ul class="ignore-global-css"> <li class="ignore-global-css">Collaborate with Veterans and other team members to identify and secure affordable housing.</li> <li class="ignore-global-css">Provide direct services to a wide range of Veterans and their families that are homeless or at imminent risk. Individual Veterans may have mental health diagnoses, physical disabilities, substance abuse challenges and other factors that may arise.</li> <li class="ignore-global-css">Responsible for weekly communications with each Veteran on the caseload.</li> <li class="ignore-global-css">Develop Housing Stability Plans (HSP) and action steps to achieve and maintain permanent housing for Veterans.</li> <li class="ignore-global-css">Work with Veterans to increase income to aid in housing stability.</li> <li class="ignore-global-css">Maintain case notes in an accurate and detailed manner.</li> <li class="ignore-global-css">Enter case notes within HMIS (Clarity) weekly for all communications with Veteran and/or collateral contacts.</li> <li class="ignore-global-css">Manage all paperwork and meet all coinciding deadlines.</li> <li class="ignore-global-css">Maintain positive and professional communications and relationships with Veterans/individuals, household members, and collateral contacts.</li> <li class="ignore-global-css">Provide ongoing referrals and resources to Veterans throughout their time in the program and document on Referral Log and case notes.</li> <li class="ignore-global-css">Completes all training within required deadlines.</li> <li class="ignore-global-css">Complies with all safety standards and practices.</li> <li class="ignore-global-css">Perform all other duties as assigned.</li> </ul>
Requirements: <p class="ignore-global-css"><strong class="ignore-global-css">SKILLS &amp; EDUCATION REQUIREMENTS:</strong></p> <ul class="ignore-global-css"> <li class="ignore-global-css">18 years of age or older</li> <li class="ignore-global-css">High School Diploma or its equivalent</li> <li class="ignore-global-css">Bachelor&rsquo;s degree or two years of equivalent experience and/or educational equivalent in working with veterans and/or with individuals who have experienced homelessness and/or are at imminent risk of homelessness.</li> <li class="ignore-global-css">Demonstrated computer literacy and capability to maintain well-organized records and files.</li> </ul> <p class="ignore-global-css"><strong class="ignore-global-css">PHYSICAL REQUIREMENTS:</strong></p> <ul class="ignore-global-css"> <li class="ignore-global-css">Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.</li> <li class="ignore-global-css">Ability to travel in and throughout the state on a quarterly basis to attend meetings, trainings, etc.</li> <li class="ignore-global-css">Successful completion of pre-employment drug screen and background check.</li> <li class="ignore-global-css">Acceptable DMV record.</li> <li class="ignore-global-css">Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a</li> <li class="ignore-global-css">TB test and/or a lift and transfer test.</li> <li class="ignore-global-css">This position requires employees to be fully vaccinated and boosted against Covid-19.</li> </ul> <p class="ignore-global-css"><strong class="ignore-global-css">Work &amp; Physical Requirements:&nbsp;</strong>This position requires the employee to regularly stand, walk, sit, talk and hear. Capable of writing, using a computer keyboard, telephone and related office equipment. Ability to lift 25lbs required. Appropriate and professional dress is expected.<br class="ignore-global-css" />When in the office, this position will normally spend long periods of time in a sitting position (60% of the day).<br class="ignore-global-css" />He/she/they may use a computer, which would involve repetitive finger, hand and wrist motion (60% of the day) and require being<br class="ignore-global-css" />able to see the computer screen (60% of the day). When in the field, other physical exertion could involve walking, climbing stairs,<br class="ignore-global-css" />lifting, stretching, bending, etc.&nbsp;</p> <p class="ignore-global-css">Appropriate and professional dress is expected.&nbsp;</p> <p class="ignore-global-css">In the field, the employee will be working in community environments, employment sites and office environments. Potential exposure to noisy environments at job sites. Exposure to blood borne pathogens could occur. (Hepatitis B vaccines are offered to all employees at no cost to the employee.)</p> <p class="ignore-global-css"><strong class="ignore-global-css">OTHER REQUIREMENTS:</strong></p> <ul class="ignore-global-css"> <li class="ignore-global-css"><strong class="ignore-global-css">Adjoin employees must be fully COVID-19 vaccinated and receive their booster dose when eligible.&nbsp;</strong></li> <li class="ignore-global-css">The Case Coordinator&nbsp;must be capable of making on-site visits to clients, agencies and employers. Often these on-site visits are at a distance from the office and no regular means of public transportation exists. Overnight travel is rare&nbsp;but may be required for training and as part of a client's program. A House Manager may transport a client as part of his/her job duties.&nbsp;</li> <li class="ignore-global-css">Adjoin does not furnish transportation or vehicles for its employees however, mileage is reimbursed. A reliable means of transportation, valid California license and insurance is required.</li> <li class="ignore-global-css">He/she/they must successfully pass a screening for illicit drug use prior to employment.</li> <li class="ignore-global-css">He/she/they must possess a record clear from past felonious criminal activity and submit to a Department of Justice criminal activity background check.</li> </ul>
