Responsibilities: <strong>Providing Direct Support involves:</strong><br /> <ul> <li>Providing the client with appropriate and effective services such as support and training programs</li> <li>Serving each client in the least restrictive environment</li> <li>Increasing the independence of all clients served</li> <li>Assisting each person with improving his/her/their quality of life</li> <li>Participating in client evaluations through task analysis and other requested assessment devices</li> </ul> <strong>Independent Living and Supported Living:</strong><br /> <br />In our Independent Living Program, Adjoin team members assist clients with a wide array of tasks to support them in their goals to be more independent, such as:<br /> <ul> <li>Meal Planning and Preparation</li> <li>Money Management</li> <li>Household Maintenance</li> <li>Shopping</li> <li>Medical Appointments</li> <li>Social Skill Development</li> <li>Volunteer Opportunities</li> <li>Safety Planning and Preparation</li> <li>Problem Solving</li> <li>Advocacy</li> <li>Transportation</li> <li>Training</li> <li>Parenting Skills</li> <li>Mental Health Symptoms</li> </ul> In the Supported Living program, Adjoin team members provide training and personal support services so that the individual may live in their own apartment or house fully integrated into the community. Traditionally, supported living services are best suited for individuals who prefer or require more comprehensive levels of regular and on-going support. This may involve assistance with personal care, home care, and meals.<br /> <br /><strong>Documentation:</strong><br /> <br />When providing direct support, Adjoin team members are responsible for writing daily activity records for our clients in our electronic client management system. This is a critical duty that helps every member of the client&rsquo;s support team communicate and collaborate to maximize the effectiveness of the services the client receives. A requirement of this position is the ability to complete this documentation within the system and meet all appropriate deadlines.<br /> <br /><strong>Transportation:</strong><br /> <br />When providing direct support, Adjoin team members must be capable of making on-site visits to clients, agencies and employers. Often these on-site visits are at a distance from the office and no regular means of public transportation exists. Overnight travel is rare, but may be required for training and as part of a client's program. This position is required to transport a client as part of their job duties.<br /> <br />Adjoin does not furnish transportation or vehicles for its employees however, mileage is reimbursed. A reliable means of transportation, valid California license and insurance is required.<br /> Qualifications Skills and Education Requirements:<br /> <ul> <li>Must be 18 years of age or older with a minimum of a high school diploma or its equivalent.</li> <li>Must successfully complete a Red Cross approved First Aid and CPR class within 45 days of the date of hire OR already possess a Red Cross approved First-Aid certificate.</li> </ul> <br />Physical Requirements: Some of our clients are physically impaired and require assistance into and out of wheelchairs and other assistive devices. The employee must be able to perform such tasks on an urgent or emergency basis. Many of the jobs/activities in which our clients engage require physical exertion on the part of the Adjoin staff person (i.e., walking (75% of workday), climbing stairs (10% workday but on a frequent basis), lifting (10% of workday but on a frequent basis), stretching, bending, etc.). The employee must have the ability to talk, hear, and see in order to assist the client in his/her/their quest for independence. Other Requirements:<br /> <ul> <li>Must be capable of making on-site visits to clients, agencies and employers. Often these on-site visits are at a distance from the office and no regular means of public transportation exists.</li> <li>Adjoin does not furnish transportation or vehicles for its employees however, mileage is reimbursed. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.</li> </ul>
