Together we ignite possibility!<br /> <br />The Case Coordinator Supervisor acts as a vital role within the Adjoin Veterans program. The purpose of this role is to provide exceptional services to the homeless Veteran population by ensuring that the policies and procedures of the case coordination team are reviewed, updated and followed. The Case Coordinator Supervisor&rsquo;s goal is to provide training, support and supervision of assigned Case Coordinator&rsquo;s and to ensure adherence to the principles of the Housing First Model in conjunction with embracing Veteran&rsquo;s choice to obtain and maintain housing stability.Review, evaluate, and determine Participant program continued eligibility approval, disapproval, and discharge. <div>&nbsp;</div>
Responsibilities: <ul> <li>Expedite temporary financial aid application packages. Review temporary financial aid case applications for eligibility, accuracy, and program recommendation.</li> <li>Review quarterly Recertification packets for continued Participant eligibility.</li> <li>Complete 30 day file audits and Exit file audit.</li> <li>Conduct regular employee supervisions and document progress, goals and challenges.</li> <li>Monitor case and program records for timeliness, accuracy and completeness.</li> <li>Coordinate and resolve program service quality matters with Participants, Case Coordinators, property owners and others as needed.</li> <li>Maintain positive communications and effective working relationships within individuals in cooperative agencies, employers, landlords, involved members of families, the public, employees and Participants.</li> <li>Train, supervise, evaluate and support employees to perform their duties in accordance with policies and procedures of Adjoin.</li> <li>Facilitate Participant interventions as needed.</li> <li>Ensure incident reports are completed and followed up per policies and procedures.</li> <li>Facilitate bi-weekly program Case Conferencing.</li> <li>Maintain and manage a minor Participant direct caseload as needed.</li> <li>Complete in a timely manner all logs, reports and Participant evaluations as required.</li> <li>Provide direct service as needed and required.</li> </ul>
