Responsibilities: <ul> <li>Collaborate with Veterans and other team members to identify and secure affordable housing.&nbsp;</li> <li>Provide direct services to a wide range of Veterans and their families that are homeless or at imminent risk. Individual Veterans may have mental health diagnoses, physical disabilities, substance abuse challenges and other factors that may arise.&nbsp;</li> <li>Responsible for weekly communications with each Veteran on the caseload.&nbsp;</li> <li>Develop Housing Stability Plans (HSP) and action steps to achieve and maintain permanent housing for Veterans.</li> <li>Work with Veterans to increase income to aid in housing stability.</li> <li>Maintain case notes in an accurate and detailed manner.</li> <li>Enter case notes within HMIS (Clarity) weekly for all communications with Veteran and/or collateral contacts.</li> <li>Manage all paperwork and meet all coinciding deadlines.</li> <li>Maintain positive and professional communications and relationships with Veterans/individuals, household members, and collateral contacts.&nbsp;</li> <li>Provide ongoing referrals and resources to Veterans throughout their time in the program and document on Referral Log and case notes.</li> <li>Completes all training within required deadlines.&nbsp;</li> <li>Complies with all safety standards and practices.</li> <li>Transport Veterans to appointments, housing searches, or other necessary locations, ensuring safety and timeliness during travel.</li> <li>Perform all other duties as assigned.</li> </ul>
Requirements: <ul> <li>18 years of age or older.</li> <li>Bachelor&rsquo;s degree or two years of equivalent experience and/or educational equivalent in working with veterans and/or with individuals who have experienced homelessness and/or are at imminent risk of homelessness.</li> <li>Certification in an approved First-Aid and CPR class; or successfully complete an approved First-Aid and CPR class within 45 days of the date of hire.</li> <li>Must successfully complete Adjoin&rsquo;s training programs as assigned.</li> <li>Insured, registered vehicle in safe working condition, and proof of current driver&rsquo;s license.</li> </ul> <br /> <p><strong>POSITION REQUIREMENTS</strong></p> <ul> <li>Ability to travel within and throughout the counties in which services are provided.</li> <li>Ability to travel in and throughout the state on a quarterly basis to attend meetings, trainings, etc.&nbsp;</li> <li>Successful completion of pre-employment drug screen and background check.</li> <li>Acceptable DMV record.</li> <li>Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.</li> </ul> <br /><br /> <p><strong>EEO</strong></p> <p>People matter.
