Responsibilities: <ul> <li>Provide community resources and referrals to meet the needs of our participants.</li> <li>Screen program applicants for eligibility via telephone intake and/or on a walk-in basis and/or during outreach services towards program&rsquo;s target populations (including street and shelter outreach).</li> <li>Assist in the design and implementation of individual training and support programs for the participants.</li> <li>Assist with completion and submission of applications for public and VA benefits for participants.</li> <li>Assist program applicants and/or participants with collecting and submitting documents as needed.</li> <li>Track screenings and participant contacts in an accurate and timely manner.</li> <li>Administer assessments to determine eligibility and appropriate interventions.</li> <li>Conduct follow along and follow up services to current and former participants.</li> <li>Provide data entry and update of databases in a complete, accurate, and timely manner.</li> <li>Maintain accurate and up-to-date participant filing.</li> <li>Maintain positive communications and relationships with his/her supervisor, participants, employers, landlords, partnering agencies, involved members of families and other individuals in the community.</li> <li>Complete in a timely manner all logs, reports and participant evaluations as required.</li> <li>Provide direct service as needed and required.</li> <li>Provides and attends all required meetings and in-services as assigned.</li> <li>Completes all required training within required deadlines.</li> <li>Complies with all safety standards and practices.</li> <li>Perform all job-related duties as assigned.</li> </ul>
Requirements: <ul> <li>18 years of age or older.</li> <li>High School diploma or its equivalent.</li> <li>Good understanding of military culture--- preferred.</li> <li>Since this position requires a great deal of interaction with people, well-developed interpersonal skills are critical.</li> <li>Demonstrated computer literacy and capability to maintain well-organized records and files.</li> <li>Ability to follow instructions is critical to this position.</li> <li>Ability to multi-task, work under pressure and manage time is also an essential component of this position.</li> <li>Certification in an approved First-Aid and CPR class; or successfully complete an approved First-Aid and CPR class within 45 days of the date of hire.</li> <li>Ability to travel for meetings, conferences, seminars and trainings.</li> <li>California insured, registered vehicle in safe working condition, and proof of current driver&rsquo;s license.</li> </ul> <br /><strong>POSITION REQUIREMENTS</strong><br /> <ul> <li>Ability to travel within and throughout the counties in which services are provided.</li> <li>Successful completion of pre-employment drug screen and background check.</li> <li>Acceptable DMV record.</li> <li>Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.</li> </ul>
