Responsibilities: <ul> <li>Conduct assessments during the participant&rsquo;s intake phase and as needed in order to understand the Veterans&rsquo; health care status, potential barriers to care, the causes, and the impact of such barriers on the veterans&rsquo; ability to access and maintain health care services.</li> <li>Connect veterans who are not already enrolled or engaged in health care to VA health care or other medical care providers</li> <li>Assist veterans in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the veterans&rsquo; care</li> <li>Work collaboratively with the Veterans multi-disciplinary team, including medical, mental health, and case and housing coordination personnel to identify and address challenges and develop solutions</li> <li>Provide comprehensive case and care coordination&nbsp;</li> <li>Act as a health coach by proactively supporting the Veteran to access and appropriately utilize treatment interventions and outcomes</li> <li>Provide education on wellness-related topics, such as healthcare plan options</li> <li>Arrange transportation to medical appointments</li> <li>Assist Veterans in preparation for medical appointments so that all issues are addressed during appointments and veterans are informed regarding health care plan</li> <li>Remain current on community medical resources available&nbsp;</li> <li>Maintain documentation of trainings, client contacts and other pertinent information in accordance with agency and best standard practices.</li> <li>Attend weekly Veteran Case Conferencing meetings to discuss Veterans with high level mental and physical health needs</li> <li>Transport Veterans to appointments, housing searches, or other necessary locations, ensuring safety and timeliness during travel.</li> <li>Other duties as assigned</li> </ul>
Requirements: <ul> <li>18 years of age or older&nbsp;</li> <li>Minimum of Bachelor's Degree in Social Work; LCSW preferred; ASW acceptable</li> <li>Master's Degree is preferred</li> <li>At least 2 years professional experience in a social service role.</li> <li>Experience in a medical/hospital environment conducting medical social work duties preferred</li> <li>Ability to rapidly develop rapport with SSVF program participants and VA medical staff and other medical teams.</li> <li>Knowledge of HMIS (Clarity) a plus.</li> <li>Prior experience working with homeless population a plus.</li> <li>Ability to prioritize competing responsibilities and thrive in a challenging and frequently changing environment.</li> <li>Excellent verbal and written communication skills</li> <li>Must be able to fluently read, write and communicate in English</li> <li>Strong analytical and problem-solving skills&nbsp;</li> <li>Well-developed interpersonal skills are critical.&nbsp;</li> <li>Demonstrated computer literacy (MS Office) and capability to maintain well-organized records and files.&nbsp;</li> <li>Ability to multi-task, work under pressure and manage time is also an essential component of this position.&nbsp;</li> <li>Demonstrated ability to effectively navigate and maintain professional boundaries as custodian of confidential information.</li> <li>Proficient skill level in Microsoft Office (Word, Excel, Power Point, Zoom, Teams, Sharepoint)</li> <li>Experience supporting programs serving homeless individuals/families and those diagnosed with physical or mental health disabilities a plus.</li> <li>Insured, registered vehicle in safe working condition, and proof of current CA driver&rsquo;s license.</li> </ul> <p><strong>POSITION REQUIREMENTS</strong></p> <ul> <li>Ability to travel within and throughout the counties in which services are provided a minimum of 25% of the time.</li> <li>Ability to travel in and throughout the state on as-needed basis to attend meetings, trainings, etc.&nbsp;</li> <li>Successful completion of pre- employment drug screen and background check.</li> <li>Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.</li> <li>Acceptable DMV record.</li> </ul> <p><strong>WORK REQUIREMENTS</strong></p> <ul> <li>This position requires the employee to regularly stand, walk, sit, talk and hear. Capable of <br />writing, using a computer keyboard, telephone and related office equipment. Ability to lift 25lbs <br />required. This position will normally spend long periods of time in a sitting position (80% of the <br />day). They may use a computer, which would involve repetitive finger, hand and wrist motion <br />(75% of the day) and require being able to see the computer screen (90% of the day). Other <br />physical exertion could involve walking, climbing stairs, lifting, stretching, bending, etc.</li> <li>Appropriate and professional dress is expected.</li> <li>The physical demands are representative of those that must be met by an employee to <br />successfully perform the essential functions of this job. Reasonable accommodations may be <br />made to enable individuals with disabilities to perform the essential functions.</li> <li>The employee will experience normal office conditions with a well-lighted work area and minor <br />noise from standard office equipment. In the community the employee will be working in <br />community environments, employment sites and office environments. Potential exposure to <br />noisy environments at job sites. Exposure to blood borne pathogens could occur. (Hepatitis B <br />vaccines are offered to all employees at no cost to the employee.)</li> <li>The AV HCC is expected to perform duties assigned during regular working hours. As necessary, <br />the AV HCC may be called upon to work in the evenings or on weekends. Unless otherwise <br />stipulated, a normal workweek will be forty hours.</li> </ul>
