Full-Time

Overview: Our full-time non-exempt (hourly) ACT Nurse LPN helps to support our mission and team so we can work in the most efficient and effective way. This position will work Monday - Friday during the core hours, with no weekend work. You may travel to other Adult &amp; Child Health locations as needed.<br /><br /><br /><br />Our Assertive Community Treatment (ACT) Team provides services necessary to house individuals who are homeless in Indianapolis. The ACT Nurse works as part of this multidisciplinary team to provide medical education and healthcare support to Program participants. The nurse completes medical assessments related to the clients&rsquo; physical and behavioral health needs, administers medication in accordance with prescriber directions, and makes referrals to community medical providers and resources. Nursing services are provided in the clinic and out in the community.
Responsibilities: <ul> <li>Gather pertinent health data and assesses the patient's general condition.</li> <li>Provide basic patient care as ordered.</li> <li>Document care provided or procedures performed accurately as well as assessments, interventions, patient/family responses, medication dispensed/prescribed, and test results in medical records.</li> <li>Educates patient/family by way of educational materials.</li> <li>Coordinates care of patients with internal and external professionals.</li> <li>Has a working knowledge of Ethics and Legal parameters.</li> <li>Administer a variety of treatments and medications as directed.</li> <li>Adheres to infection control/safety guidelines and confidentiality policies.</li> </ul>
