Our Therapists and Skills Development Specialists provide services in our consumers' homes and&nbsp;</strong><strong>other locations conducive to understanding their behavior and learning new skills . Overview: Our full-time exempt (salaried) Integrated Care Manager helps to support our mission and the work of our teams. This position is a vital part of a multidisciplinary team to address the whole health needs of individuals receiving behavioral health services. The Integrated Care Manager will work closely with behavioral health and medical providers to improve patient health outcomes utilizing evidence-based practices. You will work Monday - Friday, during core business hours, but have the opportunity for flexible scheduling. You will work out of our Indianapolis Southside Office.<br /><br /><br /> <p><strong>About Adult &amp; Child Health</strong></p> <p>Adult &amp; Child Health has served central Indiana as a Community Mental Health Center for over five decades as a 501c(3) not-for-profit organization. We&rsquo;ve been a staple in Indianapolis since 1949, originally known as Marion County Child Health Clinic. In 1982, we officially became Adult &amp; Child Health. In 2017, we became a Federally Qualified Health Center, which means we can now deliver primary care to our community. Responsibilities: <ul> <li>You will facilitate the work of multidisciplinary teams to address the whole health needs of clients who experience chronic health care conditions.</li> <li>You will work directly with the client, client&rsquo;s informal supports and providers to develop treatment plans and meet established goals.</li> <li>You will serve as the primary contact for team communication.</li> </ul>
