Overview: <p>AgeWell New York is seeking a Nurse Practitioner&nbsp;to work with members residing in Nursing Homes who are enrolled in AgeWell New York CareWell, an Institutional Special Needs Plan.<br /><br />Understanding what it takes to support the senior population across the continuum of health care needs,&nbsp;<strong>AgeWell New York Health</strong>&nbsp;stands ready with dedicated professionals and expert staff, support resources, and comprehensive care coordination to assist members optimize health care coverage.<br /><br /></p> <p>This position offers a competitive salary supported by an excellent benefit package including health insurance, 401(k), paid time off, free parking and more.</p>
Responsibilities: <p>In this position, you will work in collaboration with the other members of the Interdisciplinary Team to coordinate and support the member's functional, clinical and psychosocial needs, with an emphasis on independence and wellness.&nbsp;&nbsp;</p> <p>Supervised by the Medical Director and Assistant Director of Care Management/ISNP, the Nurse Practitioner will&nbsp;work as the liaison between the member, Nursing Home, and plan staff to develop and implement the plan of care including person-centered goal development, communicating changes in member condition and facilitating primary care preventative services.</p>
Requirements: <p>The ideal candidate will possess:</p> <ul> <li>Nurse Practitioner's License and Certification in New York State</li> <li>Registered Nurse License and Certification in New York State</li> <li>Master's degree in Nursing</li> <li>Three or more years of experience in home care, care management or a related field</li> <li>One year of experience as an Adult Educator in the health care field or an equivalent combination of relevant education and or experience</li> <li>Excellent oral and written communication and interpersonal skills, as well as exemplary analytical and research skills</li> <li>Psychiatric background is a plus</li> </ul>
