Vernon. This is an opportunity to engage deeply with clients through a community-centered, multi-disciplinary care model that promotes recovery, resilience, and independent living.</p> <h3>About the Program</h3> <p>PACT (Program for Assertive Community Treatment) is an evidence-based, intensive outpatient service designed for individuals with severe and persistent behavioral health challenges. Our team-based model includes mental health professionals, a psychiatric prescriber, nurses, vocational and substance use specialists, peer support, and care coordinators. Responsibilities: <ul> <li> <p>Provide recovery-focused services including case management, treatment planning, resource navigation, and advocacy.</p> </li> <li> <p>Conduct risk assessments and develop individualized care plans.</p> </li> <li> <p>Deliver services in the community, meeting clients where they are.</p> </li> <li> <p>Collaborate closely with the multi-disciplinary PACT team to ensure wraparound, coordinated care.</p> </li> <li> <p>Participate in 24/7 on-call rotation and crisis response.</p> </li> <li> <p>Maintain documentation and records in compliance with agency and regulatory standards.</p> </li> </ul>
Requirements: <ul> <li> <p>MA/MS/MSW in a behavioral science field.</p> </li> <li> <p>Ability to obtain WA State Agency Affiliated Counselor registration (if not already credentialed).</p> </li> <li> <p>Valid WA State driver&rsquo;s license, vehicle, and appropriate insurance (as applicable).</p> </li> <li> <p>Commitment to working evenings, weekends, and holidays as needed.</p> </li> <li> <p>Strong clinical judgment and teamwork skills.</p> </li> </ul> <h3>What Will Help You Succeed</h3> <ul> <li> <p>Experience working with individuals experiencing severe behavioral health conditions.</p> </li> <li> <p>Demonstrated ability to navigate complex systems and provide strengths-based care.</p> </li> <li> <p>Crisis management and de-escalation skills.</p> </li> <li> <p>Excellent written and verbal communication.</p> </li> <li> <p>Familiarity with electronic medical record (EMR) systems.</p> </li> </ul>
