Responsibilities: <p>As a Care Coordinator, you will:</p> <ul> <li> <p>Provide direct and ongoing support to youth and families through&nbsp;<strong>case management</strong>, treatment planning, and coordination of services</p> </li> <li> <p>Help families navigate resources, develop natural support systems, and achieve self-sufficiency</p> </li> <li> <p>Collaborate with therapists, schools, child welfare, juvenile justice, and healthcare providers</p> </li> <li> <p>Participate in&nbsp;<strong>treatment teams</strong>, provide coverage as needed, and help clients maintain stability</p> </li> <li> <p>Participate in the&nbsp;<strong>after-hours on-call rotation</strong>, which includes crisis assessment and outreach when necessary</p> </li> <li> <p>Work flexible hours in a mix of&nbsp;<strong>office, home, and community-based settings</strong></p> </li> </ul>
