Responsibilities: <p>As a core member of the IOP team, you will:</p> <ul> <li> <p>Provide evidence-based individual and group therapy, crisis planning, and case management.</p> </li> <li> <p>Conduct assessments and collaborate with clients to develop recovery-centered treatment plans.</p> </li> <li> <p>Coordinate care with other Compass Health services, primary care providers, hospitals, and community agencies.</p> </li> <li> <p>Deliver services in office, home, and community settings.</p> </li> <li> <p>Participate in a 24/7 on-call pager rotation and outreach as needed.</p> </li> <li> <p>Support clients in avoiding more restrictive levels of care through intensive, integrated services.</p> </li> </ul>
