Full-Time

Overview: The Intake Nurse is a Registered Nurse or LPN who serves as a resource person for outside referral sources and Agency staff. Assigns and coordinates cases based on patient acuity and agency staffing patterns. Processes referrals and acts to resolve problems within the department. Provides patient triage and decision making related to patient care. Provides exceptional customer service to internal and external stakeholders. Works with liaison/marketing/sales staff to assure adequate information is obtained from all referral sources. Understands the impact of insufficient information related to ICD-10 coding and productivity of the field staff.<br /><br />Understands reimbursement for home health services making measurable contributions of the improvement in Case Mix Weights.
Responsibilities: <ul> <li>Coordinates referrals and assists with case assignments.</li> <li>Will be responsible for reviewing the referral for needs of the patient and skills of the staff.</li> <li>Serves as a resource to Case Managers and Physicians concerning the Home Health availability and admission procedures.</li> <li>Supervision of Data Entry staff</li> <li>Receives incoming calls from referral sources and transcribes the information to process the referral and the admission of the patient.</li> <li>Ensures appropriate medical triage and decision making for home health care patients with acute medical problems.</li> <li>Maintains departmental computerized statistical tracking system for specific database reporting.</li> <li>Assists in the development of policies and procedures related to referral intake and data entry by making suggestions for improvement.</li> </ul>
Requirements: <ul> <li>Current license to practice as a Registered Nurse or LPN in the State of Michigan.</li> <li>Associates Degree from an accredited school of nursing is required, Bachelor&rsquo;s degree of nursing is preferred.</li> <li>Previous experience working Intake process in Medicare certified Home Health Agency required.</li> <li>Minimum of two (2) years of clinical nursing experience, home health REQUIRED.</li> <li>HCHB EMR a plus</li> <li>Supervisory skills necessary to oversee the department operations, assign cases and coordinate referrals</li> <li>Ability to multitask, organized, excellent phone skills</li> </ul>
