Overview: </strong> </span></p><p style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt; color: black;">The Hospice Licensed Medical Social Worker is responsible for consultation and direction of Social Service in Hospice. Provides orientation and education to the social service and nursing staff. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities.</span></p><p style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt; color: black;"> </span></p><p style="margin-bottom: .0001pt;"><strong><span style="font-size: 10.0pt; color: black;"><br/><br/>
Responsibilities: <ul style="margin-top: 0in;"> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Assesses the social, spiritual and emotional needs of the patient and family in order to estimate the patient&rsquo;s and family&rsquo;s ability to cope with the problems of daily living and a terminal illness.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Supervise, assess and complete Initial and Annual Competency on BSW. </span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Assists the physician and other Hospice team members to understand the spiritual, emotional and social factors related to the patient&rsquo;s health problems so that each patient&rsquo;s plan of care foster's spiritual well being and human dignity.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Assures that social service/counseling activities are integrated and coordinated within Hospice.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Ability to perform a psychosocial assessment and reassessment on all terminally ill patients. This includes pediatric, adolescent, geriatric and the general patient population.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Answers questions and offers emotional support to patient and family.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Implements the Plan of Care provisions to meet patient and family needs, which include social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Regularly re-evaluates the patient&rsquo;s and family&rsquo;s psychosocial needs.</span></li> <li style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Preform other duties as assigned.</span></li> </ul> <p style="margin-bottom: .0001pt;">&nbsp;</p> <h2 class="iCIMS_InfoMsg iCIMS_InfoField_Job">&nbsp;</h2>
Requirements: </span></strong></p><ul style="margin-top: 0in;"><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Master’s Degree from a school of Social Work accredited by the Council on Social Work Education. </span></li><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">One (1) or more years of previous experience in all aspects of social services and discharge planning.</span></li><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">A valid driver’s license, reliable automobile, and current auto insurance.</span></li><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Maintains patient confidentiality at all times.</span></li><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Attends annual review and in services.</span></li><li style="color: black; margin-bottom: .0001pt;"><span style="font-size: 10.0pt;">Attends at least 1 staff meetings, reads, and returns all monthly staff meeting minutes.</span></li></ul><p style="margin-bottom: .0001pt;"><span style="font-size: 10.0pt; color: black;"> </span></p><p style="margin-bottom: .0001pt;"><strong><span style="font-size: 10.0pt; color: black;">
