Overview: <br /> <br />In partnership with the Social Worker assigned, this position oversees and coordinates all services for children, youth and young adults. Acts as a liaison to various departments, placing agencies, community resources and affiliated agencies.<br />
Responsibilities: <br /> <ul> <li>Facilitates support groups, and training for assigned children and/or Resource Parents as required by program or contract.</li> <li>Assists in providing child and family with a structured orientation to address policies and procedures as well as questions or concerns.</li> <li>Provides relevant information to assist with appropriate treatment interventions and schedule consultations with the treatment team, as needed.</li> <li>Documents services provided to each child, youth, or young adult as well as maintains accurate logs for outside services.</li> <li>Provides necessary transportation and escorts client to court appointments, medical health screenings and other services, as needed.</li> </ul>
Requirements: <br /> <p><strong><u>Minimum Qualifications</u><span style="text-decoration: underline;">:<br /><br /></span></strong></p> <ul> <li>High School Diploma or GED.</li> <li>Must be able to operate standard office equipment and be computer literate.</li> <li>Applicant should possess excellent verbal, written, analytical and problem-solving skills.</li> <li>Must be able to communicate effectively and follow both oral and written instructions.</li> </ul> <br /> <p><strong><u>Additional Requirements</u><span style="text-decoration: underline;">:<br /><br /></span></strong></p> <ul> <li>Must possess and maintain a valid, unrestricted California driver&rsquo;s license and current vehicle insurance.</li> <li>Must be able to understand and follow policy and procedures.</li> <li>Must be able to document services, incidents, and other required forms/documents.</li> </ul> <p><br /><br />EOE, including disability/vets</p>
