Overview: <strong>Where Career Fulfillment and Work-Life Integration Meet</strong><br /><strong>Outdoor Adventure in the Grand Canyon National Park</strong><br />Elk Ridge Community Health is seeking an Advanced Practice Pharmacist to serve the community in our 340B Federally Qualified Health Center outpatient pharmacy located within Grand Canyon National Park. Responsibilities: <ul> <li>Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.</li> <li>Deliver evidence based, patient centered drug therapy recommendations and collaborate with the interdisciplinary care team.</li> <li>Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.</li> <li>Identify, prevent, and resolve medication related problems; support patient safety, quality improvement, and clinical guideline initiatives.</li> <li>Serve as the primary clinical pharmacy resource for patients, providers, and staff.</li> <li>Apply 340B program knowledge to ensure compliant, affordable medication access and sound pharmacy operations.</li> </ul>
