Overview: Elk Ridge Community Health is seeking a flexible, mission‑driven Coverage Clinician (NP/PA) for a uniquely dynamic role with no assigned patient panel. Responsibilities: <ul> <li>Provide high-quality patient care across North Country&mdash;via in‑person visits and telemedicine&mdash;with a focus on chronic disease management, preventative care, care transitions, and closing population‑health gaps.</li> <li>Provide in-person clinic coverage to support continuing service delivery during provider absences.</li> <li>Enjoy the freedom of no personal patient panel, instead supporting system needs by covering electronic inboxes and desktops for clinicians on PTO, extended leave, or who have left the organization, ensuring continuity of care and timely follow-up for patients.</li> <li>Assist during onboarding of new clinicians by managing short‑term patient needs and helping bridge care gaps while insurance enrollment and panel development are in progress.</li> <li>Collaborate with care teams, case management, and support services to strengthen care coordination and connect patients efficiently to appropriate resources.</li> <li>Use telehealth tools and the EHR effectively to communicate clearly, compassionately, and in culturally sensitive ways with patients from diverse backgrounds.</li> <li>Contribute to organizational improvement through workflow‑enhancement projects and by meeting key performance goals, such as timely follow‑ups on care gaps and rapid response to urgent results and needs.</li> </ul>
