Overview: <strong>Elk Ridge Community Health</strong>&nbsp;is seeking an&nbsp;<strong>Advanced Practice Pharmacist</strong>&nbsp;to serve as the Pharmacist in our 340B Federally Qualified Health Center outpatient pharmacy located within Grand Canyon National Park. This role offers clinical autonomy and the opportunity to practice at the top of your license while serving a rural and underserved population within a Patient‑Centered Medical Home model.<br /><br /> <p><strong>Compensation and&nbsp;Benefits:<br /><br /></strong></p> <ul> <li>Attractive total compensation package.</li> <li>Generous benefit package starting with, providing new employees 30 paid days off and 9 holidays in their first year of employment, which continues to increase with tenure.</li> <li>With a HIPSA score of 21, you may qualify for student loan repayment programs.</li> <li>Federal Tort Malpractice and Tail End Coverage.</li> <li>Paid license.</li> <li>CME funds and time-off package.</li> </ul>
Responsibilities: <ul> <li>Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.</li> <li>Deliver evidence-based, patient-centered drug therapy recommendations and collaborate with the interdisciplinary care team.</li> <li>Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.</li> <li>Identify, prevent, and resolve medication-related problems; support patient safety, quality improvement, and clinical guideline initiatives.</li> <li>Serve as the primary clinical pharmacy resource for patients, providers, and staff.</li> <li>Apply&nbsp;<strong>340B program knowledge</strong>&nbsp;to ensure compliance, affordable medication access, and sound pharmacy operations.</li> </ul>
