Full-Time

Overview: This position provides nursing care and direct service to our clients living in two of our group homes. The Licensed Practical Nurse (LPN) assures active treatment for clients, completion of personal cares (including bathing, toileting, feeding, etc.) and the achievement of personal goals and desires of our clients. Responsibilities: <ul> <li>Provide and assure active treatment in accordance with each client's individualized service plans, risk management plans, and program plans.</li> <li>Communicate with the Program Coordinator and site RN in a timely manner regarding client needs and parent/guardian interactions.</li> <li>Assure an environment that is safe, clean, comfortable and supportive for all clients served.</li> <li>Communicate maintenance needs with the Program Coordinator and initiate maintenance requests.</li> <li>Accurately administer medications and medical treatments, complete all required health &amp; medical documentation, and communicate with the site RN and Program Coordinator regarding health &amp; medical concerns. <ul> <li>Oversee and train staff on medication administration, medical equipment, and medical procedures as requested. Ensure documentation of all training.</li> </ul> </li> <li>Ensure appropriate nutrition for all clients served through preparing meals according to doctor prescribed diets or Fraser menus.</li> <li>Assist with all aspects of leisure and recreational activities and appointments, including transportation.</li> <li>Coordinate all the activities of daily living to include personal care, hygiene, and grooming.&nbsp;</li> <li>Collaborate with medical professionals to coordinate health &amp; medical needs for each client. <ul> <li>Document physician telephone orders accurately&nbsp;</li> <li>Comply with health directives from the site RN and the Health Services Manager</li> </ul> </li> <li>Assure regulatory compliance with documentation of incident &amp; accident reports and internal investigations of possible maltreatment.</li> <li>Facilitate the recognition of and response to service needs based on the cultural realities of our clients.</li> <li>Complete all training within specified timeframe.&nbsp;</li> </ul>
