Responsibilities: <p><em>This job will be expected to complete the following functions. This is not an all-inclusive list. This job may be assigned other functions or tasks as business or individual needs dictate. This is meant to be a general description of the most essential functions only.</em></p> <ul> <li>Partner effectively with Program Manager to provide leadership and oversight to the homes in the designated quad/region.</li> <li>Cover temporary absences for Program Manager with support from leadership.</li> <li>Support orientation and ongoing training of Supervised Living Coordinators, Assistant Coordinator; and DSPs as needed.</li> <li>Provide mentorship to Coordinators through regular consults, shadowing opportunities, team meeting support, and paperwork review to assure compliance with 245D requirements.</li> <li>Assess staff performance and provide timely positive and constructive feedback. Partner effectively with HR and Program Manager to address performance concerns.</li> <li>Partner with Staffing Coordinator and colleagues to manage staffing, based on the needs of individuals served and budgetary constraints.</li> <li>Develop positive working relationships with staff and support teamwork within the quad/region to increase employee engagement, reduce turnover, and encourage coverage across sites.</li> <li>Recognize opportunities for and promote collaboration within and between Supervised Living and other departments.</li> <li>Coordinate services for one dedicated Supervised Living home in accordance with 245D licensing requirements and Community Residential Settings (CRS) standards.</li> <li>Perform Coordinator duties in the absence of a Program Coordinator for both vacant positions and for various employment leaves as requested.</li> <li>Support admissions, discharges and handling of grievances, per Fraser policy and procedure.</li> <li>Assess customer satisfaction, service delivery excellence, regulatory compliance and respond to opportunities for improvement, through implementation of a quality assurance process.</li> <li>Implement responses to citations and recommendations for corrective action.</li> <li>Provide instruction and assistance to direct support staff implementing the coordinated service and support plan and the service outcomes. This involves direct observation of service delivery sufficient to assess staff competency including facilitating ongoing performance evaluation.</li> <li>Maintain accurate documentation of each individual&rsquo;s record, all employee training records, finances, staffing and quality assurance data in order to evaluate effectiveness of service outcomes.</li> </ul> <p><br />Schedule: Standard Business Hours with rotating on-call and coverage based on business need.</p>
Requirements: <p><strong>Education/ Licensure/ Certification Requirements:&nbsp;</strong></p> <ul> <li>BA/BS in a field related to human services preferred.</li> <li>Designated Coordinator qualified as defined by 245D required.</li> <li>One year of full-time work experience as a Designated Coordinator required.</li> <li>Designated Manager qualified as defined by 245D preferred.&nbsp;Please insure you know what this is before applying.</li> <li>Valid Minnesota Driver&rsquo;s License with acceptable record required.</li> </ul> <p>&nbsp;</p> <p><em><strong>Please note the health and safety of Fraser clients and staff is a top priority.</strong>&nbsp;We have created health and safety protocols to address the risk of COVID-19.
