Fraser Targeted Case Management is designed to help families navigate the complex social and medical services system and gain access to needed resources. It&rsquo;s for children birth to 21 years old who have received a qualifying diagnosis. Clients are matched with a case manager who helps them develop a service plan, supports access to different resources and services, helps coordinate services, evaluates the effectiveness of services and provides other needed support. Responsibilities: <ul> <li>Collaboratively develop Individual Family Community Support Plans (IFCSPs) based on the Diagnostic and Functional Assessments.</li> <li>Provide resources in the following areas: mental health, social, educational, health, recreational, and more depending on the client and family needs.</li> <li>Collaborate with internal and external partners in order to support clients and their families.</li> <li>Meet with each client once per month (at minimum) and check in on progress and goals.&nbsp;</li> </ul> <br /><strong>Available Locations and Schedules:<br /><br /></strong> <ul> <li>Fraser Bloomington/Virtual Office &amp; Community-Based<strong><br /></strong></li> <li>M-F, Full Time, Flexible Schedule - <span class="modified">Hybrid/</span>remote<span class="modified">.</span> Regular in person work with clients is required.</li> </ul> <strong><br /></strong>
Requirements: <ul> <li>Must be qualified in at least one of the following ways (please&nbsp;<strong><em>attach your transcripts</em></strong>&nbsp;for full evaluation of what you qualify for): <ul> <li>If degree is in social work, license is also required.</li> </ul> </li> <ul> <li>A Bachelor&rsquo;s degree or higher in one of the behavioral sciences or related fields from an accredited college or university</li> <li>Master's or other graduate degree in one of the mental health professional disciplines from an accredited college/university.</li> </ul> <li><strong>PREFERRED:</strong>&nbsp;2,000+ Clinical/Supervised hours in delivery of clinical services.</li> <li>Previous experience working with individuals ages 0-21 required.&nbsp;</li> <li>Valid Minnesota Driver&rsquo;s License with acceptable record required.</li> <li>Ability to pass MN DHS background study.</li> <li>Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.</li> <li>Effective communication in the English language, including verbal and written communication with both internal and external contacts required.</li> <li>Ability to perform basic mathematical skills, including accurate billing required.</li> <li>Ability to understand and implement all related regulations, policies and procedures required.</li> <li>Computer proficiency using Word and Excel with proficiency in keyboarding skills.</li> <li>Excellent organizational skills and attention to detail required.</li> <li>Basic mathematical skills, including the ability to add, subtract, multiply, divide and calculate percentages required.</li> </ul> <p>multiply, divide and calculate percentages required.<br /><br /></p> <p><em><strong>Please note the health and safety of Fraser clients and staff is a top priority.</strong>&nbsp;We have created health and safety protocols to address the risk of COVID-19.
