Responsibilities: In our Community Living program you will provide supervision and leadership to direct care staff working in our Supervised or Supportive living home setting. You will be providing direct cares to our clients and supervisory duties. You will comply with all regulations, policies, day-to-day operations, and budgetary parameters of Fraser's Community Living division. You will also develop and assure documentation of outcome-based recommendations, coordinated service and support plan addendums, and individual behavior/therapeutic plans for each client served, in addition to assuring accurate implementation of those plans.<br /><br /> <p><strong>Locations and Schedules:</strong></p> <p><strong>We have various locations across the metro with various schedules. Typically,&nbsp;Monday-Friday, business hours.</strong></p> <p><strong>All schedules are fulltime with benefits. Some current schedules include-</strong></p> <ul> <li>Plymouth: Monday - Friday, 9am-5pm, 7am-3pm</li> <li>New Hope: Monday - Friday, 8am-4pm</li> <li>Bloomington: Monday - Friday, 10am-6pm</li> <li>Richfield: Sunday 10am-6pm, Monday-Thursday 1-9pm</li> </ul>
Requirements: <ul> <li>A desire&nbsp;to grow within the organization</li> <li>Valid Drivers License, can pass Motor Vehicle Background Check, and have auto insurance</li> <li>Some supervisory experience</li> <li>Can qualify for Designated House Coordinator&nbsp;position&nbsp;within six months to a year.</li> </ul> <br /> <p><strong>Requirements for the Designated Coordinator position</strong>&nbsp;<em>are a combination of education and experience, as outlined by MN Statute 245D. Please read carefully before applying.</em></p> <p>Candidates must qualify based on&nbsp;<strong>one</strong>&nbsp;of the following three:</p> <ol> <li><strong>BA/BS in Human Services or related field with&nbsp;1 year of full-time work experience</strong>(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.</li> <li><strong>Associate degree in Human Services or related field with 2 years of full-time work experience</strong>(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older</li> <li><strong>A minimum of 50 hours of education and/or training related to human services and disabilities with four years of full-time work experience</strong>&nbsp;providing direct care services to persons with disabilities or persons age 65 or older.&nbsp;<strong>Note:</strong>&nbsp;Supervision must be provided by a qualifying Designated Coordinator under 245D.</li> </ol> <p><strong>Additional Requirements for all positions:</strong></p> <ul> <li>Valid driver's license with acceptable record</li> <li>Ability to&nbsp;meet DHS standards on criminal background study.</li> <li>Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.</li> <li>Excellent verbal and written interpersonal communication skills.</li> <li>Basic mathematical skills&nbsp;with&nbsp;ability to accurately manage a budget and create financial reports.</li> <li>Proficiency in using Microsoft Office programs including Excel, Word, and Outlook</li> </ul> <p>&nbsp;</p> <p><em><strong>Please note the health and safety of Fraser clients and staff is a top priority.</strong>&nbsp;We have created health and safety protocols to address the risk of COVID-19.
