This position will serve as a liaison between individuals and their family members and all Fraser clinical teams. Responsibilities: <ul> <li>Assist the child and child&rsquo;s family in obtaining needed services in the domains of mental health, social, educational, health, recreational, and related areas to improve child&rsquo;s functioning.</li> <li>Proactively assist families to identify barriers regarding access to recommended services.</li> <li>Engage with community partners and community members to build relationships.</li> <li>Provide services in a culturally responsive manner recognizing the unique and multiple cultural, medical, psychiatric, and physical needs of each individual.</li> </ul> <p><strong>Available Locations and Schedules:</strong></p> <ul> <li>Fraser Bloomington/Virtual Office &amp; Community-Based</li> <li>M-F, Full Time or Part - Time Flexible Schedule - Hybrid</li> </ul>
Requirements: <ul> <li>Must be qualified in at least one of the following ways (please&nbsp;<strong><em>attach your transcripts</em></strong>&nbsp;for full evaluation of what you qualify for): <ul> <li>If degree is in social work, license is also required.</li> </ul> </li> <ul> <li>A Bachelor&rsquo;s degree or higher in one of the behavioral sciences or related fields from an accredited college or university.</li> <li>Master's or other graduate degree in one of the mental health professional disciplines from an accredited college/university.</li> </ul> <li>4 years of Experience providing care coordination or related experience OR</li> <li>Certification as a Community Health Worker.</li> </ul> <p>&nbsp;</p> <p><em><strong>Please note the health and safety of Fraser clients and staff is a top priority.</strong>&nbsp;We have created health and safety protocols to address the risk of COVID-19.
