Responsibilities: <ul> <li>Creates and maintains participant schedules according to care plans and staff availability.</li> <li>Maintains staffing schedule to ensure coverage for sick leave, vacation, and long-term leave.</li> <li>Communicates routinely with contract agencies to maintain staffing and a long-lasting relationship.</li> <li>Assists with reconciling vendor invoices and delivering the services in the EMR.</li> <li>Coordinates Durable Medical Equipment (DME) and delivers DME items.</li> <li>Assist with semiannual home care assessments.</li> <li>Contact care providers and clients regarding day-to-day changes.</li> <li>On a rotational basis, participates in after-hours and weekend calls to handle emergencies, scheduling issues, and client referrals/intakes.</li> <li>Perform general office duties, including but not limited to word processing, photocopies, files, shreds, sort/distribute mail, provide reception coverage, telephone services, etc.</li> <li>Understand and adhere to established home.</li> </ul>
Requirements: <p><strong>Physical Demands</strong></p> <ul> <li>Ability to lift up to 30 pounds. Moving, lifting or pushing greater than 30 pounds should be done with assistance as appropriate.&nbsp;</li> <li>Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.</li> <li>Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.</li> <li>Must have high manual dexterity.</li> <li>Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.</li> </ul> <br /> <p><strong>Work Environment</strong></p> <ul> <li>Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.</li> <li>Subject to unpleasant odors.</li> <li>The noise level is usually quiet to moderate, but may at times be noisy and crowded.</li> </ul> <br /> <p><strong>Education/Experience Requirements</strong></p> <p><strong>Minimum Qualifications</strong></p> <ul> <li>Reliable transportation valid CA Driver&rsquo;s License, acceptable driving record, and vehicle insurance.</li> <li>Must be able to work effectively in a team environment.</li> <li>Only act within the scope of his or her authority to practice.</li> <li>Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, be flexible, innovative, focus on results and travel regularly.</li> <li>Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.</li> <li>Meet a standardized set of competencies established by Central Valley PACE and approved by CMS before working independently.</li> <li>Proficiency with Microsoft Suite and/or Google Suite, and scheduling software required.</li> <li>Bilingual English / Spanish.</li> </ul> <br /> <p><strong>Education/Experience</strong></p> <ul> <li>High school diploma, or equivalent.</li> <li>Current BLS CPR Card certified by American Heart Association.</li> <li>Minimum two (2) years' caregiver experience.</li> <li>Minimum one (1) year of administrative experience.&nbsp;</li> </ul>
