Overview: <p>Supports mission, vision, and values of Golden Valley Health Centers Central Valley PACE Center by acting as the Lead Patient Care Assistant and delivering patient care activities and tasks as assigned to assist the nursing staff.&nbsp;<strong>THIS IS A HOME CARE POSITION.&nbsp;</strong></p> <p><strong>This role will be trained at our Modesto center.</strong></p> <p>Schedule: Monday - Friday, 8am - 5pm.&nbsp;</p> <p>Department: Home Care</p> <p><strong>Compensation:&nbsp;</strong>$24.31 - $25.53 an hour</p> <p><strong>This position requires travel throughout all of Merced County.</strong></p> <p>Golden Valley Health Centers offers excellent benefits including Medical: (0 Deductible / $2,000 Individual; $4,000 Family Out-of-Pocket Max), excellent PPO coverages; Dental; Vision; 403(b) with match, FSA plans, gym discounts, and so much more!</p>
Responsibilities: <ul> <li>Acts as the mentor to PCAI.</li> <li>Communicates patient care needs to the LVN, supervising RN and update Manager.</li> <li>Assists in training PCA&rsquo;s and other staff.</li> <li>Perform COVID and Influenza test when assigned.</li> <li>May assist in Day Center and Activities departments when there is down time.</li> <li>Deliver DME to PRT&rsquo;s home and document in EMR.</li> <li>Assist with Home Safety Assessments when assigned only when there is down time on the schedule.</li> <li>Flexible to work weekends when needed.</li> <li>Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.</li> <li>Communicates effectively with participants, families, staff, other departments and agencies.</li> <li>Works effectively in a team setting.</li> <li>Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.</li> <li>Participates in teaching/learning process with others.</li> <li>Follows guest relations policies and procedures.</li> <li>Abides by all established policies, rules and regulations.</li> <li>Attends staff meetings and takes part in participant care planning.</li> <li>Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.</li> <li>Communicates effectively with participants, families, staff, other departments and agencies.</li> <li>Works effectively in a team setting.</li> <li>Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.</li> <li>Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.</li> <li>Performs patient care activities appropriately, reporting any circumstances or change in patient&rsquo;s condition.</li> <li>Other projects and duties as assigned.</li> </ul>
Requirements: <p><strong>Physical Demands<br /><br /></strong></p> <ul> <li>Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.&nbsp;</li> <li>Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.</li> <li>Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.</li> <li>Must have high manual dexterity.</li> <li>Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.</li> </ul> <br /> <p><strong>Work Environment<br /><br /></strong></p> <ul> <li>Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.</li> <li>Subject to unpleasant odors.</li> <li>The noise level is usually quiet to moderate, but may at times be noisy and crowded.</li> </ul> <br /> <p><strong>Education/Experience Requirements</strong></p> <p><strong>Minimum Qualifications<br /><br /></strong></p> <ul> <li>Valid California Driver&rsquo;s License, acceptable driving record and vehicle insurance.</li> <li>Must have reliable transportation.</li> <li>Must be able to treat people of all ages including geriatrics.</li> <li>Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.</li> </ul> <br /> <p><strong>Education/Experience<br /><br /></strong></p> <ul> <li>High School Diploma or equivalent required.</li> <li>Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.</li> <li>Current BLS CPR Card certified by the American Heart Association.</li> <li>Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.</li> </ul> <br />
