Overview: <p>The Quality Improvement Specialist is responsible for providing support to the organization's quality improvement plan through the implementation of targeted quality improvement projects and performance improvement activities.</p> <p>This position will be based at our Atwater Administration Site. Responsibilities: <ul> <li>Demonstrates problem solving skills and effective communication, both verbally and in writing.</li> <li>Works with health plans through the QI Project Manager (Panel Management) as required/needed to review and address gaps in incentive goals.</li> <li>Coordinates implementation of activities for incentive deliverables including calling patients, sending letters, scheduling appointments, etc.</li> <li>Works collaboratively with other quality improvement staff including QI Project Managers and QI Coaches as well as operational management to implement projects that enhance patient care.</li> <li>Works collaboratively with other QI staff and operational management to promote Quality and safety projects.</li> <li>Collaborates with local site staff to provide needed training and competency assessments for specific to quality and safety activities.</li> <li>Supports the quality improvement department in the auditing of workflows as it relates to the documentation of quality indicators.</li> <li>Collaborates and supports QI coach teams with trends, care gap reports, and other data in relation to quality metrics.</li> <li>Generates accurate and timely QI data.</li> <li>Enters data timely and accurately, into established databases, spreadsheets and other electronic medium as needed.</li> <li>Completes regular analysis of QI data and relays findings to QI Project Manager.</li> <li>Submits and presents reports to QI team about the progress in metrics across the quality improvement plan and health plan incentive programs.</li> <li>Participates as a preceptor or trainer during general staff training presentations for quality improvement activities as directed by QI Project Manager.</li> <li>Proactively assumes responsibility for continuous improvement activities.</li> <li>Willingness to explore and implement innovative quality programs/projects.</li> <li>Data entry, gathering, and analysis for QI grants, projects, and programs.</li> <li>Demonstrates understanding of the mission, vision and goals of the health center.</li> <li>Other duties and projects as assigned.</li> </ul>
Requirements: <p><strong>Physical Demands &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</strong></p> <ul> <li>Lift up to 30 pounds occasionally and push up to 100 pounds (on wheels) on rare occasions.</li> <li>Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.</li> <li>Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.</li> <li>Must have high manual dexterity.</li> <li>Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.</li> </ul> <br /> <p><strong>Work Environment &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</strong></p> <p>The noise level in the work environment is usually quiet to moderate.<br /><br /><br /></p> <p><strong>Minimum Qualifications:</strong></p> <ul> <li>Proficient with MS Office &ndash; Word, Excel and PowerPoint.</li> <li>Knowledge of Electronic Health Records (EHR), immunization registry, quality improvement concepts and performance measurements.</li> <li>Follow guidelines from QI Manager and accurately executes the steps for process improvement.</li> <li>Relate in a professional manner and handle confidential information.</li> <li>Successfully navigate paper based and electronic health records.</li> <li>Connect and build strong professional rapport.</li> <li>Highly proficient in medical terminology.</li> <li>Excellent communication, written and verbal, and strong comprehension skills.</li> <li>Valid drivers&rsquo; license, vehicle insurance and acceptable driving record.</li> <li>Ability to meet deadlines, prioritize tasks and interact effectively with various levels of staff and departments.</li> <li>Ability to collect, correlate and analyze data.</li> <li>English fluency required.</li> </ul> <br /> <p><strong>Education/Experience Required:</strong></p> <ul> <li>High school diploma or equivalent.</li> <li>Associate Degree or Medical Assisting Certificate from an accredited program.</li> <li>Two (2) years&rsquo; experience in health care required, in the area of quality improvement, preferred.</li> </ul>
