Six core values unite us: Champion Health, Practice Humility, Embrace Change, Drive Impact, Learn &amp; Grow and Be Heartfelt.</p> <p><strong>Who we need</strong></p> <p>We need someone who can take inspiration from those who live with heart disease, stroke and vascular cognitive impairment - survivors and their families - to create and disseminate powerful messages that inspire system change at the local, provincial and national level; someone who will fight to make space for the voices that might not otherwise be heard - those in rural, remote, transient communities, indigenous people and women across the country.</p> <p>Reporting to the Director, Systems Change and Stroke Program and working closely with internal Provincial Leads, we are looking for a Senior Manager, Systems Change to make a difference carrying out our strategy and executing positive changes that will enhance health systems of care to meet the needs of people in Canada living with heart disease, stroke and vascular cognitive impairment.</p> <p><strong>What&rsquo;s in it for you?</strong></p> <p>You want to play an instrumental role at the grassroots level. You want to help implement our strategy, our research and our knowledge in real and tangible ways within our communities; not just with healthcare providers and administrators but with the people who have a lived experience with heart disease, stroke and vascular cognitive impairment. You want to synthesize emerging evidence and trends into content and action items for internal teams and external stakeholders across every province. You want to play a critical role in advancing change within our health systems, through evidence, advisory services, advocacy and education.</p> <p><strong>Who you are</strong></p> <p>You are genuinely invested in improving the lives of people in Canada living with heart disease, stroke and vascular cognitive impairment. A project manager, consultant and strategic leader, you can build strong internal and external relationships, influence decisions and create pathways of possibility and adoption. You can influence and inspire provincial leads, guiding their work and providing coaching and project support.</p>
Responsibilities: <p><strong>How you will make an impact every day. You will focus on:</strong></p> <ul> <li>Development of an evidence-based, dynamic, responsive, flexible, and scalable framework for assessing and monitoring provincial systems of care to support people with heart disease, stroke and vascular cognitive impairment. You will: <ul> <li>Develop a comprehensive approach to bringing about systems change, ensuring women and indigenous health priorities are included in the framework at every level.</li> <li>Conduct ongoing assessments and updates of information on provincial stroke system models, provincial cardiac system models and the care and coordination of people living with heart disease, stroke and vascular cognitive impairment, including community and social services, environmental services, and strategies addressing the needs of the aging population.</li> <li>Create and disseminate reports on system activities back to multiple stakeholders.</li> <li>Leverage technology to drive systems change and ensure current information is available on systems infrastructure.</li> </ul> </li> <li>Implementation of strategic priorities across provinces and territories. You will: <ul> <li>Work with the Director Systems Change and Stroke Program, the Director of Data, Knowledge Management and Heart Program and the provincial mission leads to create and implement comprehensive, strategic initiatives from Mission Critical Area councils and other Mission inputs.</li> <li>Identify and establish relationships with key stakeholders internally and externally to support change initiatives and implementation of strategic projects.</li> <li>Work closely with the Policy, Advocacy and Engagement,&nbsp;Data and Knowledge Development teams to ensure an integrated and coordinated approach.</li> <li>Build opportunities to advance Indigenous health and women&rsquo;s heart and brain health strategy.</li> </ul> </li> <li>Information management related to health systems change. You will: <ul> <li>Maintain up-to-date knowledge of trends in health systems, management, prevention and public health fields as they relate to H&amp;S priorities.</li> <li>Collaborate with the Senior Manager, Data and Knowledge to respond to requests for information and generate reports in a timely manner.</li> <li>Ensure accurate and effective translation of knowledge on systems and stroke into easy to understand materials for a non-technical audience (e.g. donors, people with lived experience, caregivers, and corporate partners).</li> </ul> </li> <li>Knowledge sharing and promotion through activities and resources for health professionals. You will: <ul> <li>Supervise and work closely with the Senior Specialist, Knowledge Exchange for Health Professionals (KEHP).</li> <li>Manage and support the development and implementation of a knowledge exchange framework relevant to external stakeholders and healthcare provider audiences.</li> <li>Work with the Senior Specialist, Knowledge Exchange for Health Professionals (KEHP) and the Conference Planning and Logistics Team, mission leads, and other functional leads to plan, deliver and evaluate the scientific programs for the Heart &amp; Stroke Clinical Update, Canadian Stroke Congress and hosted sessions at the Canadian Cardiovascular Congress.</li> <li>Oversee the delivery and evaluation of three annual lectureships.</li> <li>Identify health care audience needs and implement opportunities to further develop and enhance professional education competence.</li> <li>Ensure all program content is aligned and meets accreditation standards.</li> <li>Leverage technology and innovative approaches to advance our role and reach.</li> <li>Collaborate with stakeholders and partners in assessing target audience learning needs and preferences.</li> <li>Provide leadership to cross-functional teams to ensure integrated provincial dissemination of national heart disease, stroke and vascular cognitive impairment material.</li> </ul> </li> </ul>
Requirements: <p><strong>What you bring:</strong></p> <ul> <li>The education. You have a Master&rsquo;s degree in health administration, public health, health policy, life sciences, applied science or social science. You have coursework or related education in best practices for systems change. Member of a registered health profession is an asset.</li> <li>The language. You are fluent in English and ideally in French as well.</li> <li>The experience. You have 7 years in the health sector, including demonstrated success working with ministries of health and other government departments and agencies. You have experience: <ul> <li>Designing and developing systems change frameworks and in implementing systems initiatives.</li> <li>Facilitating knowledge translation and exchange, ensuring up-to-date education for health professionals.</li> <li>Disseminating complex information and concepts to broad multi-stakeholder audiences.</li> <li>Working with people who have experienced heart disease, stroke and vascular cognitive impairment. (Not required but a significant benefit.)</li> </ul> </li> <li>The innovation. You have a proven ability to synthesize, interpret, analyze and report on data and scientific evidence. You are strategic with proven critical thinking. You can assess and create plans to mitigate risk, communicate issues and provide realistic solutions.</li> <li>The knowledge. You have strong knowledge of business processes. You have the analytical ability to work with data and present results to non-technical audience.</li> <li>The communication and relationship skills. You can adapt approaches in light of stakeholder needs/priorities and develop appropriate strategies. You can provide confident consultative advice across the organizations and with external stakeholders at the community level as well as the administration level.</li> <li>The technical skills. You are comfortable with MS Office, SharePoint, a CRM and workflow tools to support and advance the work of the team.</li> <li>The project management abilities.
