The Emergency Department at Henry Ford West Bloomfield Hospital is a Community-based non-teaching ED situated within the beautiful West Bloomfield campus.  The ED sees over 40,000 patients per year and boasts 32 private rooms including 1 Mental Health, 1 Ophthalmology and 6 Fast Track rooms, 2 Trauma Bays and an adjacent 14-bed Clinical Decision Unit.

Overview: <p>HFWBH is a Primary Stroke Center, offers a 24-hour Cardiac Cath Lab and the ED is currently seeking Level III Trauma Verification.&nbsp; We are dedicated to serving our community through growth, continual process improvement and providing team-based emergency care. Responsibilities: <ul> <li>The Social Worker l is responsible for creating outcomes for the patient and family by managing complex psychosocial and economic co-morbidities.</li> <li>Provide education and consultation regarding resources, collaborate to develop a discharge plan and facilitate and monitor its implementation.</li> <li>Assist patient/family adjust to and cope with illness, diagnosis, treatment options and prognosis.</li> <li>Through advanced practice skills mobilizes resources to reduce risk, serves as a liaison between the hospital and the community.</li> <li>In addition offers crisis interventions to patient/family.</li> <li>36 hours/week Midnight Shift</li> </ul>
Requirements: <ul> <li>Master&rsquo;s Degree in Social Work (LMSW)</li> <li>Up to two years social work or protective services experience, required.</li> <li>&nbsp;Working knowledge/experience in utilization management, managed care and payor issues, preferred.</li> <li>Experience in psychosocial and therapeutic counseling, 1 year, preferred.</li> <li>Ability to interview, assess, organize and problem solve.</li> <li>Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multidisciplinary team and community agencies to achieve desired patient outcomes.</li> </ul>
