Full-Time

Overview: <ul> <li>The primary services to be provided to the client and other family members involve family preservation and support services, including assessments.&nbsp;</li> <li>Effectively providing services to families defined as resistant to services.</li> <li>Provide emergency and crisis consultant on a 24-hour basis.</li> <li>Provide individual and/or family sessions according to the client&rsquo;s treatment model.</li> <li>Engage and assist the family&rsquo;s participation in the treatment planning process.</li> <li>Provide sessions and service hours in accordance with FCT requirements.&nbsp;</li> <li>Visit in client&rsquo;s home no less than two times per week.</li> </ul>
Responsibilities: <ul> <li>Provide services with a variance of days/hours designed to respond to the client/family needs and schedule.</li> <li>Provide structured, goal-oriented, time-limited services with families who need assistance in recovering from physical, sexual, emotional abuse, neglect, and trauma-related events. Other issues may be present and may need to be addressed in the course of treatment.</li> <li>Adhere to the FCT treatment model, fidelity measures, service hours, and deadlines.</li> <li>Accompany and support family for court appearances.</li> <li>Provide closure and aftercare plan for each client/family.</li> <li>Completion of clear, accurate documentation of 1) all contacts with clients and collaterals, 2) monthly reports, 3) treatment plans, and 4) court reports.&nbsp; Provide additional written reports to the referring agency as requested.&nbsp;</li> <li>Completion of treatment model fidelity documents for each phase or type of services.&nbsp;&nbsp;</li> <li>Complete all clinical and agency paperwork in a timely and accurate manner.</li> <li>Comply with required in-service / continuing education hours as outlined in policy manual.&nbsp;</li> <li>Attain/maintain CPR and First Aid Certification.</li> <li>Provide Adoption &amp; Guardianship Support Services as needed and as assigned.</li> <li>Ensure that services are provided Effectively, Efficiently, and with Evidence for each case</li> <li>Contribute to Lifeline's Performance and Quality Improvement process.</li> <li>Maintain professional demeanor, boundaries, dress, communication, style, overall manner.&nbsp;</li> <li>Address all risk/crisis management areas with the supervisor/Lifeline Crisis Line to ensure safety.</li> <li>Maintain strict confidentiality at all times to protect clients' privacy.</li> <li>Demonstrate respect and cultural sensitivity to all clients and to all referral sources.</li> <li>Participate in collateral meetings as appropriate for the services being provided to the client.</li> <li>Actively build and maintain positive collateral relationships two or more times per week.</li> <li>Participate and engage in weekly team meetings, individual supervision, and trainings.</li> </ul>
