Overview: We&rsquo;re seeking a skilled Director, Health and Wellness to oversee the personal care staff and coordinate all resident care services in our Assisted Living and Memory Care communities. You&rsquo;ll monitor resident health, ensure compliance with care plans, and lead a team committed to the safety, wellbeing, and quality of life of every resident. Responsibilities: <br /> <ul> <li>Perform comprehensive health assessments on new residents upon admission</li> <li>Monitor health status of all residents through assessments and care conferences</li> <li>Review and ensure staff compliance with care plans, medication protocols, and resident needs</li> <li>Coordinate and attend resident care conferences and service plan reviews</li> <li>Respond immediately to suspected resident abuse and report per policy</li> <li>Supervise Assisted Living and Memory Care personnel, including hiring, coaching, training, scheduling, and performance evaluations</li> <li>Address employee relations issues and implement verbal and written disciplinary actions as needed</li> <li>Attend daily management and staff meetings, resident council meetings, and family events</li> <li>Conduct community audits with the Executive Director and Regional Clinical team to monitor compliance and ensure community security</li> <li>Review incident reports, investigate, and implement corrective actions</li> <li>Ensure Continuous Quality Improvement (CQI) activities meet community goals</li> <li>Foster teamwork, provide guidance to staff, and maintain positive resident and co-worker interactions</li> <li>Promote a safe work environment and compliance with safety standards</li> <li>Perform other duties as assigned</li> </ul> <br />
Requirements: <br /> <ul> <li>Associate’s or Bachelor’s Degree in Nursing strongly preferred; Licensed Vocational or Practical Nurses may be considered based on state regulations</li> <li>Minimum three years’ experience in Assisted Living, Alzheimer’s, or Memory Care preferred</li> <li>Proven management experience and ability to lead staff</li> <li>Experience as Residential Care Manager, Clinical Director, or in Risk Management preferred</li> <li>Licensed or Registered Nurse required with state-specific accreditation or license</li> <li>First Aid certification required</li> <li>Strong written and verbal communication skills</li> <li>Self-motivated, organized, detail-oriented, and able to work independently</li> <li>Ability to evaluate staff performance and provide coaching and training</li> <li>Compassion for and desire to work with older adults</li> <li>Ability to understand and communicate effectively with residents, families, supervisors, and co-workers</li> <li>Proficiency with Microsoft Word and Excel preferred</li> <li>Must meet health requirements, including TB clearance, and pass required background checks</li> </ul> <br />Physical Requirements<br /> <ul> <li>Ability to stand, walk, sit, use hands, reach, stoop, kneel, crouch, or crawl</li> <li>Ability to talk, hear, and use vision effectively</li> <li>Occasional weekend, evening, or night work and on-call as needed</li> <li>Possible exposure to communicable diseases, blood-borne pathogens, latex, and chemicals</li> <li>Ability to lift and assist residents as needed, up to 50–100 lbs occasionally</li> <li>Travel may be required occasionally</li> </ul> <br />
