Overview: Provides direct care to residents while promoting their highest level of functioning as directed by management. Assists in maintaining a safe and clean environment. Responsibilities: <ul> <li>Receive and give report to management on resident status at the beginning and end of each shift and as needed</li> <li>Notify management of any changes in resident physical and/or mental status</li> <li>Provide personal care to residents per individualized plan of care</li> <li>Document all pertinent information regarding care</li> <li>Prepare residents for meals, assist to and from dining room, and help with meal setup and eating</li> <li>Assist residents with toilet and incontinence care</li> <li>Make room checks and verify resident location as directed</li> <li>Keep residents&rsquo; apartments neat and clean</li> <li>Perform clinical procedures according to facility standards and scope of practice regulations</li> <li>Perform vital signs, blood pressure, weights, and other trained procedures</li> <li>Complete prevention and restorative care including ambulation, transferring, bathing, grooming, and all other Activities of Daily Living</li> <li>Maintain knowledge of facility infection control procedures</li> <li>Assist in turning, positioning, and repositioning residents as outlined in the service plan</li> <li>Participate in Interdisciplinary Team Process including documentation of care and attending care plan meetings</li> <li>Communicate effectively with residents, families, visitors, healthcare team members, and other disciplines</li> <li>Actively participate in facility&rsquo;s Quality Assurance Program</li> <li>Report elder maltreatment or abuse to Director of Health and Wellness, Executive Director, and/or State</li> <li>Maintain education, compliance with regulations, and current knowledge of resident care practices</li> <li>Follow facility emergency procedures including fire and disaster drills</li> <li>Maintain resident confidentiality and adhere to HIPAA policies</li> <li>Perform other duties as assigned</li> </ul>
Requirements: <ul> <li>Certification as a CNA from an approved State course</li> <li>Six months&rsquo; experience in long-term care preferred</li> <li>Meet all health requirements, including TB testing</li> <li>Pass state-required fingerprinting and criminal background check</li> <li>Compassion for and desire to work with the elderly</li> <li>Skilled in conflict resolution and active participation in resident care assessments</li> <li>Demonstrates leadership, communication, and coaching skills</li> <li>Strong written, verbal, and interpersonal skills</li> <li>Excellent organizational skills, detail-oriented, and able to manage time effectively</li> <li>Pleasant, professional, and personable manner</li> <li>Ability to understand and communicate effectively with residents, guests, supervisors, co-workers, and vendors</li> </ul>
