Responsibilities: <ul> <li>Nurse practitioner will function under the supervision of a physician.</li> <li>This provider will be expected to provide clinical services both face to face, and telemedicine visits based on access and patient demand.</li> <li>This position is for a certified NP that has at least 2 years of post-training OB/GYN practice experience.</li> </ul> <br /> <p><strong><u>Specific Accountabilities</u></strong>:</p> <ol> <li> <ol> <li><strong>Primary Care Coverage</strong> <ol> <li>Assigned to a specific MMG site to provide face to face clinical care, telemedicine and resolve clinical messages under the supervision of a MMG physician.</li> <li>Clinical care for patients as a primary care provider, including the assessment, evaluation, and management of patients including but not limited to:&nbsp; <ol> <li>Assess patient health by interviewing patients and reviewing patient histories.</li> <li>Performs physical examinations.</li> <li>Determines health status and abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, laboratory studies and interprets test results.</li> <li>Performs therapeutic procedures such as suture removal, administering injections and immunizations.</li> <li>Accurately documents patient care services by charting in electronic medical record in a timely manner.</li> <li>Provides continuity of care by developing and implementing patient management plans</li> <li>Instructs and counsels patients by explaining clinical assessments and therapeutic regimens.</li> </ol> </li> <li>Clinical activities will include preventive, chronic disease, and acute care management.</li> <li>Participates in site&rsquo;s clinical QI/ PI activities during days/ hours at the site</li> <li>Administrative responsibilities: includes Epic in basket coverage as expected for primary care providers including but not limited to: <ol> <li>Forms</li> <li>Phone calls</li> <li>Results</li> <li>Medication refills</li> <li>MyChart messages</li> <li>Staff messages</li> <li>cc&rsquo;d charts</li> </ol> </li> <li>Clinical and administrative responsibilities will support patients and physicians of the entire practice.</li> </ol> </li> </ol> </li> </ol>
Requirements: <ul> <li>New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN</li> <li>Current New York State Nurse Practitioner specialty certification and registration</li> <li>Master&rsquo;s or post Master&rsquo;s Certificate in specialty as a nurse practitioner preferred or a combination of education and experience that is acceptable to NY State Education Department and the Director of Nursing and/or Chairman of the Department, or Division Director</li> <li>National certification in nurse practitioner specialty (ANCC, AACP, or NAPNAP) preferred</li> <li>Effective English communication skills, both written and oral</li> <li>BLS certification</li> </ul>
