Responsibilities: <ul> <li>Act as liaison between patient, family, and physician</li> <li>Provide updates on the patient condition to family and physician</li> <li>Ensure all in-home medical equipment are in working order</li> <li>Document all details of patient&rsquo;s condition, progression and regression activity</li> <li>Oversee required physical therapy or rehabilitation exercises</li> <li>Contact emergency services when patients need to be taken to the hospital</li> <li>Possess knowledge of Home Care reimbursement process</li> <li>Possess knowledge of OASIS</li> <li>Have reliable transportation as you go to/from home visits</li> <li>Can walk up/down six (6) flights of stairs as needed</li> </ul>
