Overview: <br />This vacancy will be posted on internal Career Website for 5 business days.<br /> <br />Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.<br /><br /> <br />We are currently seeking Physician Assistants to join our esteemed UBA-WeCARE Program, a subsidiary of Montefiore Health System. WeCARE provides a continuum of services to help New York City &ndash; Department of Social Services &ndash; Human Resources Administration (NYC -DSS- HRA) referred cash assistance clients with medical and/or mental health conditions that affect their employability to attain their maximum levels of self-sufficiency. Individuals referred to WeCARE receive medical and psychosocial assessments to identify possible clinical and mental health conditions and social barriers that may affect their ability to participate in work-related activities. Based on the results of the assessments, a determination of an individual's functional capacity (FCO) is made and a customized service plan by our team is developed to provide a range of services tailored to meet a client's particular needs. An individual will be referred for a medical evaluation performed by a Physician Assistant only in certain circumstances, such as when a client disagrees with a functional capacity outcome (FCO) determination or when an FCO cannot be determined from the assessment. The Physician Assistant will function independently with physician and Medical Director oversight. Responsibilities: <ul> <li>Obtaining medical histories</li> <li>Performing physical examinations</li> <li>Writing medical evaluation reports</li> <li>Interpreting diagnostic tests</li> <li>Formulating medical and mental health diagnoses</li> <li>Evaluating the need for work limitations or accommodations</li> <li>Making determinations of functional capacity with written justifications</li> <li>Provide clinical guidance for Social Security Administration disability applications, that are prepared by WeCARE Staff</li> <li>Provide medical history summary support letters for clients applying for disability with the Social Security Administration</li> <li>Retrieve medical documentation from electronic medical record systems such as EPIC</li> </ul> <br /> <br />
