Overview: <p>The AIDS Center provides HIV primary and specialty care, sexually transmitted disease screening and treatment, HIV biomedical prevention services, Hepatitis C care, and LGBTQ care at 2 ambulatory sites, the Center for Positive Living/ID Clinic and the Oval Center, and the inpatient consult services. The sites serve approximately 4,000 patients per year at about 35,000 visits and are steadily growing.</p> <p>To meet the access, medical, and biopsychosocial needs of the AIDS Center's complex populations living with or at high risk for HIV, Hepatitis C, and sexually transmitted infections, the AIDS Center leadership has committed to increasing access to comprehensive, culturally appropriate care according to best practices for destination clinics as outlined by NYC DOHMH. Responsibilities: <p>The Chronic Care Registered Nurse is a key member of the AIDS Center staff, responsible for monitoring the LPNs and PCTs as needed, providing direct nursing care/assessment/triage/medication injections and dispensing, health education, and managing the transitions of and outreach to medically complex patients in conjuction with the patient educators. The Chronic Care Registered Nurse will be cross trained to provide clinical support at the Center for Positive Living/ID Clinic and The Oval Center, although their primary site will be the Center for Positive Living/ID Clinic.</p> <p>&nbsp;</p> <br /><br /> <br /> <br />
Requirements: <ul> <li>Bachelor's Degree required, Master's degree preferred</li> </ul> <ul type="disc"> <li>New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an R.N.</li> </ul> <ul type="disc"> <li>3 years Clinical experience with at least 1 year of Ambulatory experience required</li> </ul> <ul type="disc"> <li>Basic Life Support Training</li> <li>Infection Control Training</li> <li>Certification in specialty preferred</li> <li>Effective English communication skills, both written and oral</li> <li>Ability to use basic data entry and retrieval systems</li> </ul> <br /><br /> <br /> <br />
