Overview: The Outreach Specialist for the High-Risk Care Management Program (HRCM) will engage with patients prior to discharge across Moses, Wakefield and Weiler Campuses- location may vary based on need. The HRCM program is intended to assist high acuity patients transition from inpatient to outpatient care.&nbsp;
Responsibilities: <ul> <li>The Outreach Specialist will aid patients in the program complying with scheduled appointments and will help patients engage and comply with their onsite and offsite psychiatric treatment.</li> <li>When needed, the Outreach Specialist will perform home visits to foster engagement.</li> <li>The Outreach Specialist will aid in initial patient engagement, outreach efforts and accompanying patients to appointments as needed.</li> <li>The HRCM program is a component of the Behavioral Health Centers of Excellence award.&nbsp;</li> <li>This is a 100% grant funded position. This position requires travelling to other locations based on departmental needs</li> </ul>
Requirements: <ul> <li>High School Diploma/GED required, Bachelor's degree preferred</li> <li>Possess a certification from, or are provisionally certified as, a New York Certified Peer Specialist by an OMH-approved Certified Peer Specialist certification program;</li> <li>Excellent oral, written and interpersonal communication skills. Ability to find creative solutions to challenges. Familiarity with Microsoft Word and Excel.</li> <li>Peer Specialists/Advocates who hold a credential from a certifying authority recognized by the Commissioner of the Office of Addiction Services and Supports (Certified Recovery Peer Advocate) are eligible to work in MHOTRS programs provided they qualify for and obtain provisional OMH Peer Certification or Credentialing within 12 months of being hired. It is expected that, within a reasonable amount of time, they will then complete full OMH Peer Certification or Credential their work experience hours post provisional certification/credentialing</li> </ul> <br /> <br /> <br />
