Responsibilities: <ul> <li>This position will provide evidence-based HIV prevention, risk reduction education and capacity building to individuals and grassroots/community-based organization (GR-CBO) partners to improve the GR-CBO&nbsp;</li> <li>Partner&rsquo;s capacity to implement social networking and other proven engagement strategies for HIV self-test kit distribution and PrEP education/uptake; supports the provision of referrals for essential and supportive&nbsp;</li> <li>Services, ensures linkage to care, (escort from a community-based location to Montefiore Health System for clinical follow-up), supports recruitment for MPP&rsquo;s Community Engagement Group, intervention activities, and condom distribution;&nbsp;</li> <li>Responsible for eligibility screenings, intake and data entry for services provided, aimed at the programmatic goal of improving the health of MSM, LGBTQ+ and high-risk individuals in the Bronx and NYC, not limited to other job responsibilities</li> </ul> <br /> <br /> <br />
Requirements: <ul> <li>Excellent interpersonal, communication and oral skills / Excellent public speaking to make presentations</li> <li>Proficiency in engaging the community and patients / Ability to communicate health education messages</li> <li>Knowledge of computer programs and proficiency in Microsoft Word, Excel and Power Point (Will also use photocopier and fax machine, as needed)</li> <li>Willingness and ability to work a flexible schedule to meet the needs of the community</li> <li>&nbsp;Second language a plus</li> </ul> <br /> <p>Hours: Monday to Friday 1000A &ndash; 0630P<br />Education: Bachelor&rsquo;s Degree</p> Experience:<br /> <ul> <li>At least 1 year of experience in HIV prevention and LGBT+ community&nbsp;</li> <li>Knowledge of care coordination and local community resources</li> </ul> <br /><br /> <br /> <br />
