Overview: Montefiore Medical Center is proud to announce the upcoming launch of a new acute inpatient psychiatric facility dedicated to providing high-quality, short-term psychiatric care for children and adolescents. This specialized unit will enhance access to timely, developmentally appropriate mental health services, offering a safe, structured, and compassionate environment to support patients in crisis and help stabilize their condition.<br /> <br /> As part of our efforts to build a strong, multidisciplinary team, we are currently seeking a Social Worker. As an integral member of the interdisciplinary treatment team, the Inpatient Social Worker provides a comprehensive range of direct and indirect services to patients and their families. Responsibilities: <ul> <li>Conduct comprehensive psychosocial assessments for all assigned patients, including history, strengths, risk factors, and social determinants of health.</li> <li>Provide short-term therapeutic interventions including supportive counseling, crisis intervention, psychoeducation, individual therapy, family therapy, and group treatment</li> <li>Participate actively in treatment planning with the interdisciplinary team, contributing clinical insights and advocating for patient needs.</li> <li>Develop and implement individualized discharge plans in collaboration with the patient, family, and care team.</li> <li>Coordinate referrals and linkages to community services including outpatient mental health providers, housing, substance use treatment, higher levels of care and social support services.</li> <li>Communicate regularly with outside agencies and post-discharge providers to ensure continuity of care.</li> <li>Performs and documents weekly assessments throughout patient's stay.</li> <li>Serve as a liaison between the patient, family, treatment team, and external agencies.</li> <li>Facilitate family meetings and treatment conferences as needed.</li> <li>Collaborate with legal, protective, and government agencies in cases involving guardianship, child/adult protective services, or court-mandated treatment.</li> <li>Participate in departmental meetings, case conferences, and ongoing professional development.</li> <li>Adhere to ethical and professional standards in accordance with NASW guidelines and hospital policies.</li> </ul>
Requirements: <ul> <li>Master&rsquo;s Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education required.</li> <li>LMSW (Licensed Masters Social Worker) or LCSW (Licensed Clinical Social Worker) or limited permit required</li> <li>Understanding of developmental stages from infancy through elder years, family systems theory, and how various illnesses impact patient / family system</li> </ul> <br /> <br /> <br /> <br /><br />
