The nurse conducts comprehensive assessments of the family&rsquo;s physical, emotional, social, and environmental needs and provides counseling, health education, referrals, and case management related to prenatal and postpartum care, nutrition, parenting, child health and development, family planning, and life-course development.</p> <p>The nurse-client partnership is the foundation for change, as the nurse supports clients in clarifying personal goals for themselves and their children and developing the skills needed to achieve those goals.</p>
Responsibilities: <ul> <li>Conduct home visits in accordance with NFP Model Elements and Visit-to-Visit Guidelines</li> <li>Apply the nursing process to implement the NFP model in practice</li> <li>Provide education, support, and referrals to assist women and families in achieving targeted goals</li> <li>Collaborate and consult with interdisciplinary professionals involved in family services</li> <li>Formulate nursing diagnoses based on assessments and client-identified goals</li> <li>Plan and sequence home visits considering client acuity, geographic location, and program priorities</li> <li>Evaluate client progress toward stated goals and NFP-defined outcomes</li> </ul>
Requirements: <ul> <li>Baccalaureate Degree in Nursing</li> <li>Licensed and currently registered to practice as a Registered Nurse in New York State</li> <li>Minimum of two (2) years of recent experience in maternal-child health</li> <li>Public/community health or home visiting experience preferred</li> <li>Current CPR certification required</li> <li>Excellent written and verbal communication skills</li> <li>Basic computer proficiency</li> <li>Valid driver&rsquo;s license and insured automobile required</li> <li>Ability to work a flexible schedule, including some evenings and weekends, per agency requirements</li> </ul>
