Overview: <p>For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.</p> <p>&nbsp;</p> <p>The Montefiore Licensed Practice Nurse contributes standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing&rsquo;s Social Policy Statement.<br /><br />The LPN actualizes the vision, mission, values, balanced score card performance measures for Montefiore Medical Center. Participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. Demonstrates core and unit specific competencies based on the patient population served.<br /><br />Contributes to the actualization of the vision, mission, values and philosophy and balanced score card performance measures for the Medical Center. He/she is committed to maintaining the standards of the practice of nursing and is responsible for the delivery of care to the neonate, pediatric, adolescent, adult and/or elderly patient within their scope of practice. The LPN participates in performance improvement, research activities and maintains clinical expertise.</p>
Responsibilities: <ul> <li>Provide direct care for a population specific patient assignment reflective of current and status of patient.</li> <li>Provide patient/family teaching and discharge instructions to meet learning/discharge needs as identified by RN.</li> <li>Set priorities, problem-solve, and use proper judgment in difficult situations and be flexible in non-emergent situations.</li> <li>Provide and maintain safe environment for patients/families through preventative and interventional measures.</li> <li>Document patient care per policies and procedures.</li> <li>Participate, promote, and practice continuous quality improvement. Collect and document of data relevant to planning/evaluation of patient care.</li> <li>Communicate effectively with patients/families and members of the health care team.</li> <li>Demonstrate medication administration as per MMC policy and procedure.</li> <li>Responsible and accountable for own practice.</li> <li>Demonstrate body mechanics as it relates to lifting/transfer/procedural assistance and knowledge and skills necessary to provide care appropriate to patient population served.</li> <li>Use Blood pressure, temperature, breathalyzer, pulse oximeter equipment, electronic clinical information system, and perform phlebotomy.</li> <li>Responsible for maintenance of Customer Service.</li> <li>Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.</li> <li>Utilize a teaching and discharge plan based on documented patient/family learning needs, chronological maturation and cognitive ability.</li> </ul>
Requirements: <ul> <li>High School Diploma/GED required, Associate Degree preferred.</li> <li>New York State License for Practical Nurse/LPN &amp; Basic Life Support Certification are required.</li> <li>Effective English communication skills, written, oral and interpersonal required.</li> <li>Ability to use basic data entry and retrieval system required.</li> <li>Additional education required by specialty areas/patient care locations.</li> </ul> <br /><br /> <p>This is a 100% grant funded position.</p>
