It is responsible for clinical procedures, patient intake and rooming, medication and immunizations, lab duties, patient education and care coordination. This position is 100% grant funded.</p>
Responsibilities: <ul> <li>Provide hands-on care under RN/physician supervision, including administering medications, monitoring vitals, wound care, triaging/rooming patients within 15 minutes, and performing treatments.</li> <li>Accurately document patient info, vitals, interventions, and medication administration with 100% compliance. Collect, label, and log specimens (blood, biopsy) with zero tolerance for error.</li> <li>Clean and stock exam rooms between sessions, maintain supply par levels, and sterilize equipment via autoclave.</li> <li>Partner with the healthcare team to escalate issues, facilitate discharge, and teach patients/families regarding care management.</li> <li>Provides direct care for a specific patient population. Administers medications and treatments. Provides patient &amp; family teaching and discharge instructions.</li> </ul>
Requirements: <ul> <li>NY LPN License &amp; Registration.</li> <li>BLS Card and Infection Control Certification.</li> <li>High School/GED minimum.</li> </ul> <p>&nbsp;</p> <p><strong>Shift</strong>: Monday to Friday 0900A &ndash; 0530P; Saturday rotation with weekday off; Occasional late shift of 1030A &ndash; 0700P or 1100A &ndash; 0730P<br /><br /><br /></p> <p>This vacancy will be posted on internal Career Website for 5 business days.</p> <p>Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.</p>
