Responsibilities: <ul> <li>Provides comprehensive social work services including discharge planning and coordination, patient education and counseling to assist in meeting the psychosocial and discharge planning needs of patients and families.</li> <li>Enables patients to utilize medical care in order to achieve their optimal level of health.</li> </ul>
Requirements: <ul> <li>Master's degree in Social Work required.</li> <li>NYS License in Social Work required.</li> <li>Minimum of one year of experience as a Social Worker required.</li> <li>Discharge planning experience.</li> <li>Knowledge of Medicare Regulations.</li> <li>Understanding of Managed Care Environment.</li> <li>Excellent Interpersonal and Communication Skills.</li> <li>Case Manager Certification a plus.</li> </ul>
