Responsibilities: <ul> <li>Management of patient population to optimize utilization.</li> <li>Discharge planning.</li> <li>Coordinate with physicians, nurses, physical therapy and ancillary staff to facilitate all departmental processes.</li> <li>Communicate with Home Health Agencies and Managed Care Plans, provide clinical information for admission and follow-up patient reviews.</li> <li>Ensure payer documentation and information requirements are met.</li> </ul>
Requirements: <ul> <li>New York state License for Registered Professional Nurse and current registration or valid permit to practice in New York state as an R.N.</li> <li>Bachelor's degree required.</li> <li>Minimum of 2-4 years of experience required in an acute care setting.</li> <li>Knowledge of Medicare Regulations.</li> <li>Excellent interpersonal and communication skills.</li> <li>Experience with electronic health records.</li> <li>Case Management Certification preferred (CCM).</li> </ul>
