Overview: For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.
Responsibilities: <ul> <li>Responsible for screening all patients for medical necessity of admission, regardless of point of entry, with the use of industry approved Level of Care Criteria.</li> <li>Provides the care and services of patients across the continuum of their hospitalization.</li> <li>The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient&rsquo;s treatment and discharge needs, which would achieve optimal clinical and resource outcomes.<strong>&nbsp;</strong></li> </ul>
Requirements: <ul> <li><strong>Education/Training</strong>: Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write, and communicate in English.</li> <li>&nbsp;<strong>Experience</strong>: Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational, and interpersonal skills.</li> <li>&nbsp;<strong>Licensure:</strong>&nbsp;Current New York State Licensure as a Registered Professional Nurse.&nbsp;Certification in Case Management required 18 months after eligibility. PRI certification preferred.&nbsp;</li> </ul>
