Overview: Demonstrates understanding of assessment, planning, and managing patient care. <br />
Responsibilities: <ul> <li>Current knowledge of trends and techniques in patient care.</li> <li>Familiarity with all state and regulatory requirements related to case management and social work.&nbsp;&nbsp;</li> </ul>
Requirements: <ul> <li>Bachelor's Degree required.&nbsp;</li> <li>Master's Degree preferred.&nbsp;</li> <li>Minimum of seven (7) years' related experience in a healthcare setting, with at least five (5) years in a management or leadership capacity.</li> <li>Current NYS Licensure as a Registered Professional Nurse or Social Worker required.</li> <li>Certification in Case Management, PRI Certification preferred.</li> </ul>
