Overview: <strong>**This position is in the Navigation Department </strong><br /><br />This position plays a pivotal role in guiding patients through the complexities of managing health conditions.
Responsibilities: <ul> <li>The role focuses on improving patient adherence to treatment plans, ensuring coordinated care, and enhancing overall health outcomes.</li> <li>Provides personalized education, emotional support, and resources to empower patients in self-managing their health.</li> <li>Collaborating closely with physicians, specialists, and other healthcare providers, this position helps ensure that patients receive comprehensive, effective, and continuous care throughout their health journey.</li> </ul>
Requirements: <ul> <li>BS Graduate from an accredited nursing program. Required</li> <li>1-3 years Two (2) years of RN experience in ambulatory care Required</li> <li>Additional experience in medical surgical inpatient unit Preferred</li> <li>1-3 years experience in patient relations, education or customer service-oriented position in the healthcare field Required</li> <li>Ability to read, write and communicate in English.</li> <li>Ability to manage details, people, and multiple projects.</li> <li>Demonstrated verbal and written communication skills required.</li> <li>Microsoft Word and Epic experience is required.</li> <li>Microsoft Excel and PowerPoint experience preferred.</li> <li>Registered Nurse New York - New York State Board of Nursing Required</li> <li>BLS - American Heart Association Required</li> <li>Breast Navigation Certification Preferred</li> </ul>
