Overview: <p>For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.</p> <p><strong>** This position will be in the Emergency Department, Variable Shifts </strong></p> <p>Responsible for ensuring interdisciplinary pharmaceutical care programs are appropriately integrated throughout the institution.</p>
Responsibilities: <ul> <li>Participate in all aspects of the medication use system, including formulary management, clinical surveillance, and clinical policy/protocol/guideline development and implementation.</li> <li>Provide comprehensive pharmaceutical care in assigned areas. &nbsp;</li> <li>Champion and operationalize quality and regulatory compliance initiatives designed to improve medication use and pharmacy practice.</li> <li>Assist in the continuous oversight and growth of the residency program.</li> <li>Provide medication and practice-related training and education to pharmacy team, members of the patient care team, and patients.</li> </ul>
Requirements: <div id="requirements" class="job-summary-section ng-scope"> <div class="full-text ng-binding"> <p><strong>Education/Training</strong>: PharmD required. PGY1 in applicable specialties strongly preferred. Ability to read, write and communicate in English.</p> <p>&nbsp;</p> <p><strong>Experience:</strong> Greater than 3 years of hospital experience as a clinical pharmacist/clinical pharmacy specialist/clinical coordinator/advanced practice pharmacist preferred. Clinical experience with antimicrobial stewardship, anticoagulation stewardship, and/or opioid stewardship preferred. Residency Program management experience preferred.</p> <p>&nbsp;</p> <p><strong>Licensure, Registration or Certification: </strong>Current New York State pharmacist license/registration renewal required.
