Overview: <p>For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.</p> <p>&nbsp;</p> <p>&nbsp;</p> <p><strong>**This is a PD Saturday Position </strong></p>
Responsibilities: <ul> <li>Coordinates the care and services of patients across the continuum of their hospitalization.</li> <li>The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.</li> </ul>
Requirements: <ul> <li>BSN Graduate of NLN Accredited School of Nursing Required or</li> <li>MS Master's Degree from an accredited school of Social Work Required.</li> <li>1-3 years of Experience in an acute care environment Required.</li> <li>Ability to read, write, and communicate in English.</li> <li>Strong clinical, organizational, and interpersonal skills.</li> <li>Registered Nurse New York - New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse required.</li> <li>Licensed Clinical Social Worker - New York State Board of Social Work within 180 Days Required.</li> <li>CCM - Certified Case Manager - Commission for Case Manager Certification. Certification in Case Management within 1-1/2 Years Required.</li> <li>PRI - Patient Review Instrument Cert - PHTC PRI certification Preferred.</li> </ul>
