Responsibilities: The Nurse Practitioner (NP), Primary Care NP Led Virtual Clinic, will provide comprehensive primary care virtual services for Central Region for patients accessing Health811. Part of a larger integrated primary health care team at OVH, the NP will provide virtual patient consultation and assessments within the scope of the primary care NP for patients of all ages and demographics who book an appointment. This role involves a strong leadership component with specific focus on initiative, communication, integration of care and follow up to help mitigate barriers to access for care and help deter ED visits. This role will include medical assessment, treatment and evaluation, education and health promotion, disease and injury prevention, acute medical treatment, and ability to make connections to the next level of care or follow up as required. The NP will need advanced knowledge and decision-making skills to provide a comprehensive health focused approach in meeting the needs of patients. The NP can work collaboratively with a team to determine next steps for patients in ensuring appropriate transitions in care and follow up. The NP will work collaboratively with the hospital physicians and community partners caring for patients in the community. The NP will work closely with the Physician Lead for EYRND OHT and its associates in establishing clinical pathways and escalation processes.<br /> <br />
